SAFETY RULES

(HIGH VOLTAGE RULES)

1997

These rules prescribe the safety practices which shall apply to ALL persons when working on or in the vicinity of
electrical apparatus where the potential difference between the conductor, or conductors and earths, of 650V and
above and forming or capable of forming part of any power supply distribution network supplied by electrical
apparatus which is the responsibility of the Public Transport Corporation.

Strict adherence to these safety practices is necessary to ensure:

a. Safety of employees and others. This shall be the prime consideration under all circumstances.

b. Continuity of service.

c. Welfare of plant.

This manual supersedes all previous issues of the PTC Electrical Safety Rules (High Voltage Rules), and issued under
the authority of the Group Manager, Infrastructure Power, of the Public Transport Corporation.

Managers are responsible for ensuring that staff within their control (including works contracted) are trained to the
appropriate level for the task and adhere to the rules. Interpretation of the rules is delegated to the Manager Train
Electrical in the first instance. Where the Manager, Train Electrical is not able to resolve the matter it is to be referred
to the Group Manager Infrastructure Power who is the final arbiter.

SECTION ONE - INTRODUCTION
1. APPLICATION

1.1 These rules prescribe the safety practices which shall apply to ALL persons when working on or in the
vicinity of high voltage electrical apparatus forming or capable of forming part of any power supply
distribution network supplied by electrical apparatus which is the responsibility of the Public Transport
Corporation.

1.2 Each Group and Department manager of train electrical infrastructure, shall be responsible for maintaining
the overall integrity of electrical installations and systems within their Group or Department including those
contracted out for services and ensure that staff are trained to the appropriate level for the task. All training is
to be reported to the Manager Train Electrical who will ensure records of all staff electrical training is
maintained.

1.3 Interpretation of the rules is delegated to the Manager Train Electrical in the first instance. Where the
Manager Train Electrical is not able to resolve the matter it is to be referred to the Group Manager
Infrastructure Power. The Group Manager Infrastructure Power is the final Arbitrator where the rules or their
interpretation are brought under question and shall resolve any disputes involving the Rules and operations.

1.4 The Group Manager, Infrastructure Power, through the Manager Train Electrical, is responsible for the issue
of Qualification Certificates and/or documents.

1.5 These Rules may be elaborated on and supplemented by Departmental instructions or company instructions
in the case of contractors which, except for special circumstances, shall not relax the principles laid down
herein.

1.6 Any Supplementary Instructions prepared by a manager for his area of responsibility area must be authorised
by the Group Manager, Infrastructure Power.

1.7 Should any work area find it essential to authorise the temporary relaxation of any of these principles, the
manager of that area may do so only for a specific occasion and only provided that other precautions or
procedures do not increase risk to employees or others.

1.7.1 The manager will personally advise the Train Electrical Manager of the details of the occasion.
The Train Electrical Manager will advise the High Voltage Field Coordinator and the respective
Electrical Systems Officer if he approves of the measures adopted.

1.7.2 If the Train Electrical Manager does not approve of the measures, he will advise the Group
Manager, Infrastructure Power and seek his direction on the matter.

1.7.3 If the need continues for more than 24 hours, the manager must seek approval of the Group
Manager, Infrastructure Power.

SECTION TWO - TERMINOLOGY
Explanation of Terms Used in this Document.

TERM DESCRIPTION
Alive Refer to ‘Live’

Apparatus Any electrical equipment, including overhead lines and underground cables, the
conductors of which are live or can be made live.

Apparatus Switching A certificate issued by an Authorised Electrical Operator to employees of


Certificate (FRM 6.1) another organisation confirming the condition of apparatus during the currency
of the Certificate.

Application to Work in the Form requesting to work in the vicinity of overhead equipment.
Vicinity of Overhead Equipment
Approved Device A device that has been approved by the Group Manager, Infrastructure Power,
tested to an approved standard and endorsed for a specific function.

Approved Person, Substation An Approved Person, Substation, is a person who is at least 18 years old (except
for substation apprentices) and who has been trained and tested in the knowledge
of hazards associated with apparatus installed in the Corporation's traction power
and industrial supply locations.
Approved Persons, Substations, under the direction of a competent person or an
Authorised Electrical Operator, are authorised to enter traction power and
industrial supply locations. Under no condition are they to approach within one
metre of exposed live High Voltage electrical apparatus unless instructed by a
Competent Person or Authorised Electrical Operator.

Approved Person, Overhead An Approved Person, Overhead, is a person who is at least 18 years old (except
for Overhead apprentices) and who has been trained and tested in the knowledge
of hazards associated with apparatus installed in the Corporation's Overhead
locations.

Authorised Person, Substation An Authorised Person, Substation, is a person who is at least 18 years old
(except for substation apprentices) and who has been trained and tested in the
knowledge of hazards associated with apparatus installed in the Corporation's
traction power and industrial supply locations.
Authorised Persons, Substations, are authorised to enter traction power and
industrial supply locations. Under no condition are they to approach within the
limits of approach of exposed live High Voltage electrical apparatus unless
instructed by a Competent Person or Authorised Electrical Operator.

Authorised Electrical Operator An Authorised Electrical Operator is a person who has passed an examination
approved by the Train Electrical Manager or his delegate and authorised by the
Group Manager, Infrastructure Power to carry out switching, isolating, earthing
and short-circuiting and issue and cancel permits and certificates on electrical
apparatus. See Clause 2.1 for details.
An Authorised Electrical Operator who has been suitably trained and has been
approved by the Train Electrical Manager may be authorised by the Group
Manager, Infrastructure Power, to perform tests on live apparatus and issue and
cancel the Testing Permit and certificates. See Clause 2.2 for details.

Caution Notice A notice which is placed on switches or other means of isolation for equipment,
to indicate that the state of the equipment is not to be changed. Refer to AS
1319, "Safety signs for the Occupational Environment".
Circuit Breaker A circuit breaker is a device for making, carrying and breaking load currents and
fault currents. It is capable of automatic opening under predetermined fault
conditions.
Competent Person A person not less than 18 years old who is competent and authorised to receive
Electrical Access Permits and assist Authorised Electrical Operators in defined
operating procedures and work areas. See Clause 2.4 for details. By definition
an Authorised Operator is a Competent Person.
Corporation The Public Transport Corporation of Victoria.

Danger Notice A notice which is used in conjunction with screens, and/or ropes, and/or barriers,
and/or safety observers to indicate to persons working in the vicinity of live,
exposed electrical apparatus that the apparatus is live and is NOT safe to
approach closer than the limits specified in Clause 4.1 Refer to AS 1319, "Safety
signs for the Occupational Environment".

De-energised Not live from normal source, may become live due to lightning, leakage,
induction, accidental contact or incorrect operation of apparatus. Note that
apparatus in this condition must be treated as live.
Discharged Connected to the general mass of earth or short-circuited in such a manner as to
remove any residual electrical energy.

Earthed Connected to the general mass of earth and short circuiting of electrical
conductors in the case of AC circuits so as to permit an effective discharge of
electrical energy.

Connected to the negative conductor or to the power rail(s) of the electric


traction system or to a structure which is connected to the power rail of the
electric traction system in the case of 1500V DC traction system so as to permit
an effective discharge of electrical energy.

Earthing and Short-Circuiting An approved device used for earthing and short-circuiting of electrical
Device conductors.

Electrical Access Permit A form of authorisation which allows access to work on isolated and
(FRM 6.2) earthed and short-circuited high voltage apparatus. See Clause 6.2 for details.

Electrical System The control centre from which all electrical operations of the Corporation’s
Control Centre (Electrol) train High Voltage electricity power supplies are controlled.

Electrical Systems The Officer responsible for all operations of the train traction,
Officer industrial and signal supply systems and supplies. This includes the 1500V DC
traction system, as well as the electrolysis drainage system for trains.

Employee Any officer or employee of the Public Transport Corporation or those deemed to
be an employee by statutory regulations such as an employee of a contractor to
the PTC.

Engineer in Charge of An engineer in charge of a section or nominee, responsible for the


Apparatus apparatus.

Exposed Electrical Apparatus Electrical apparatus, the approach to which is not prevented by a barrier of rigid
insulating material which is adequate for the voltage concerned or by an earthed
metal screen.

Hand Earth Short-circuit An approved earthing and short-circuiting device not incorporating any stick or
other insulated means of application and therefore necessitating manual contact
with the earthing and short-circuiting conductor during the process of
application.

High Voltage Difference of potential normally existing between conductors and conductors and
earth exceeding 1000V for AC and 1500V for DC as defined by AS 3000-1991.
For the purposes of these rules and PTC work this is defined as voltages of 650V
and above.

In Commission Capable of being made alive by the closure of switches, circuit breakers or
isolators.

Industrial Supply Electrical Power supply for purposes other than traction and signal power supply.

In the Vicinity of Electrical At such distance from electrical apparatus where there is reasonable concern
Apparatus that the clearances listed within these rules may be breached. For exposed
electrical equipment installed on the Corporation’s train infrastructure power
supply network that clearance is 2 metres for persons (including hand-held
objects) and 6.4 metres for mobile plant (see Annex I), unless otherwise defined
in clause 4.

In-Service Equipment or apparatus which is available for performing its normal function.

Isolated Separated from all normal sources of supply by operation of isolators or their
equivalent such as racking out and locking open. Isolated apparatus may become
live due to leakage, induction, etc.
Isolator A non-load breaking device which when opened provides the appropriate
designed clearances from electrical conductors to prevent accidental or
inadvertent energisation of some part of the circuit.

Live Having a voltage difference between conductors or to earth. All apparatus must
be treated as live unless earthed or short circuited.

Live Line Detector An approved device for determining whether electrical apparatus is live or not
live.

Live Line Work Work carried out on live 1500V DC overhead conductors and apparatus in
accordance with instructions issued by the Manager Train Overhead.

Operating Stick A stick designed, approved and tested for performing operations on live
apparatus. See Clause 8.4

Out of Commission Disconnected from all sources of supply by the physical removal of a portion of
each of the conductors concerned and providing a break at least equal to the
relevant one specified in Clause 3. See Clause 4.2 for details.

Out of Service Not available for service but not necessarily out of commission or isolated.

Permit to Work A form of authorisation to permit work to be carried out in the vicinity of, but not
(FRM 6.5) in contact with high voltage apparatus which has been isolated, earthed and /or
short circuited. For details see Clause 6.5.

Persons authorised to Persons who have been approved by the Engineer-in-Charge of electrical
receive Permits to Work apparatus to receive a Permit to Work.

Recipient in Charge A recipient in charge is a Competent Person and a representative of the work
party for the work (or the major portion of the work) and who arranges for all
persons involved in access to the apparatus to sign on and off the permit form as
appropriate.

Safety Observer An employee suitably instructed and specifically assigned the duty of observing
and warning against unsafe approach to electrical apparatus.

Short-circuited The accidental or intentional connection by a low resistance path between two or
more points in an electrical circuit.

Statement of Conditions of A certificate issued by another Network Operator which states that specified
Apparatus and/or Plant apparatus and/or plant under it's control is in the condition described and
will remain so during the currency of the certificate. A document equivalent to
the Corporation's Apparatus Switching Certificate.

Stick Earth and Short-circuit An approved earthing and short-circuiting device which is fitted with stick or
Device sticks of insulating material enabling the operator to apply the earthing and
short-circuiting device at the prescribed distance from and without touching the
apparatus being earthed and short-circuited.

Stick Operated Disconnects An approved device operated by suitably insulated stick or sticks, which is used
for the purpose of providing a removable connection between different circuits
or sections of AC transmission lines.

Stick Operated Jumpers An approved device operated by a suitably insulated stick or sticks, which is
used for the purpose of electrically connecting two 1500 volt DC overhead
conductors.

Switch A switch is a device capable of making and breaking load currents. A switch
may also serve as an isolator.

Testing Permit (EE 399) The form of authorisation which allows access to electrical apparatus for testing
purposes. See Clause 6.3 for details.

SECTION THREE - ELECTRICAL SAFETY RULES
1. SAFETY WARNINGS
The following paragraphs draw attention to a number of hazards which are associated with electrical apparatus and to
some of the precautions which can be taken to avoid such hazards.

WARNING

FAILURE TO OBSERVE THESE PRECAUTIONS


MAY LEAD TO SERIOUS INJURY OR DEATH.

1.1 Danger of Contact with or Close Approach to Exposed Live Electrical Apparatus

Serious injury or death from electric shock may result from contact with or close approach to exposed live electrical
apparatus. Do not contact or approach live exposed apparatus with any part of the body or with any object other than an
appliance designed and approved for the purpose. The only exception shall be when working in strict accordance with
the procedures laid down under live line working on 1500V apparatus in accordance with the instructions of the
Overhead Engineer.

1.2 Personnel Rescue

When a person comes into, and remains in contact with live electrical equipment, do not attempt rescue unless you are
trained in live line rescue techniques and have the necessary equipment to do so. Immediately contact the Electrical
Systems Officer to arrange isolation or de-energisation of equipment and wait for confirmation of de-energisation before
attempting to move the victim.

The Electrical Systems Officer will co-ordinate the rescue operation.

Employees who consider they can do so safely, may open a circuit breaker, switch or isolator to rescue a victim and then
advise the Electrical Systems Officer of the situation and request assistance.

1.3 Fire, Smoke from High Voltage Apparatus

Breakdown of electrical equipment may result in fire and issue of a large amount of smoke.
Fire or electrical breakdown of some types of electrical insulation may also result in release of toxic fumes.

Fire extinguishers and hoses are not designed for use on live high voltage equipment.

Immediately contact the Electrical Systems Officer to arrange de-energisation and/or isolation of equipment.

1.4 Fire Extinguishers

Fire extinguishers which are marked "Suitable for Use on Electrical Fires" are intended for use on circuits of a voltage
lower than those encountered in rail traction, signal and industrial power supply installations.

Fire extinguishers or fire hoses of any kind MUST NOT be used on live electrical apparatus in Rail traction,
Signal and Industrial power supply installations.

1.5 Use of Measuring Devices

Only mechanical or electrical measuring devices of approved material and/or design shall be used in the vicinity of
electrical apparatus.

a. Folding steel rules, metal tapes, linen tapes interwoven or reinforced with metal thread, wooden rulers with
metal edges, or any other conductive device SHALL NOT be used in the vicinity of electrical equipment and
apparatus.

b. Only approved electrical meters, instruments and testing equipment (including live line detectors) shall be
used on the Corporation's High Voltage equipment and apparatus.

1.6 Ladders

Wire reinforced ladders or metallic ladders shall not be used on or in the vicinity of electrical apparatus.

1.7 Unnecessary Approach to Electrical Apparatus

Unnecessary approach to apparatus shall be avoided at all times. Unnecessary contact with metallic items associated
with the equipment such as transformer tanks, overhead structures, frames, covers etc. shall be avoided. When necessary
the approach to electrical apparatus shall be restricted to the period required to perform the work.

1.8 Handling Long Objects

When objects of a length liable to cause a hazard are being handled manually or by mechanical equipment, care shall be
taken to prevent the objects or the equipment violating mandatory clearances.

1.9 Radio Transmitters and Receivers

Radio receivers and transmitters with extended aerials shall not be used in the vicinity of the apparatus except under
strict supervision.

1.10 Movement of Live Cables

The general rule is that live insulated cables shall not be moved nor shall work be performed in their vicinity in such a
manner as may disturb the insulation. Movement of live cables is to be avoided except when absolutely necessary for
operational and safety reasons and then only with the permission of the Engineer in Charge for the asset and in
accordance with the requirements set out in Clause 9.3 of these rules.

1.11 Work in Tunnels and Confined Spaces

In tunnels and similar confined spaces containing electrical apparatus, whether exposed or insulated, an unrestricted
means of escape shall be maintained preferably on each side of the position from which the work is being performed.
The relevant Codes of Practice must be complied with.

1.12 Use of Cranes, Mobile or Portable Equipment

When cranes, mobile or portable equipment is used in the vicinity of live apparatus the possibility of inadvertent contact
with that apparatus shall be taken into account. See Appendix I for requirements.

1.13 Personal Protection

Employees working in the vicinity of electrical apparatus shall be suitably clothed by complete body covering including
sleeves worn to wrist length and trousers worn to ankle length in order to avoid flash burns/shocks or other injury. They
shall wear approved safety boots or shoes, appropriate gloves and eye protection. Protective gloves are worn when
working on Overhead and approved insulated gloves are worn when operating High Voltage apparatus. In defined
places and while performing specified operations they shall also wear an approved safety helmet.

1.14 Rubber Gloves

Rubber gloves shall not be used as the sole means of insulation when working on high voltage apparatus of a higher
voltage than 1000V AC. There is the requirement to use approved insulating gloves and approved insulating mats when
High Voltage Switching in the High Voltage AC network, as detailed in Clause 1.18 below.

1.15 Live Apparatus

In addition to direct and normal connection to the supply, it should be noted that apparatus may become live through
other means such as by:
a. accidental connection to normal or other supply by various means,

b. inductive or capacitive coupling from adjacent circuits, or

c. lightning.

1.16 Current Transformer Secondary Circuits

The secondary circuit of a current transformer shall not be open circuited whilst in service, as a dangerous voltage may
be induced on the secondary circuit.

1.17 Interference with Earthing or Negative Connections


Interference or disconnection of earthing or negative cables installed on electrical apparatus may result in serious injury
or death.

1.18 High Voltage Switching

If during the switching process for the high voltage AC network and contact with electrical apparatus incorporating a
metal handle is unavoidable, Departmental procedures approved by the Group Manager Infrastructure Power shall
ensure that the operator is suitably protected.

2.1 Authorised Electrical Operator

Persons required to perform switching, isolating, earthing, short-circuiting of apparatus and to issue and cancel
Electrical Permits and Certificates on electrical apparatus must be Authorised Electrical Operators with a level
of authorisation that is appropriate to their needs and work area. See Clause 2.3.

They shall be at least 18 years of age, be a qualified Competent Person, have passed an examination in PTC
Electrical Safety Rules and be appropriately trained to perform those switching operations which are
associated with their normal duties.

They shall be in possession of a current First Aid Level 1 certificate or equivalent so that they have been
instructed and examined on resuscitation and external cardiac massage as well as methods of release of victims
from contact with live apparatus.

After having satisfied the Train Electrical Manager or his representative as to their knowledge and ability to
operate on electrical equipment in a given Work Area, they shall be authorised in writing by the Group
Manager, Infrastructure Power.

The form of authorisation is shown in Appendix "A".


The Group Manager, Infrastructure Power may withdraw or suspend an Authorisation at any time.

Normally the authorisation of Electrical Operators is restricted to the Work Area in which they are usually
employed and to a level where they have satisfied the Train Electrical Manager or his representative as to their
knowledge and ability. The Work Area, levels and any special restrictions will be specified on the
Authorisation Certificate issued to them.

Authorised Electrical Operators shall be issued with and acknowledge receipt of a copy of the Electrical Safety
Rules. They shall remain thoroughly conversant with the rules and may be required to demonstrate
competency to the Train Electrical Manager or his representative. They shall be responsible for keeping their
copy of the Rules up to date.

2.2 Authority to Conduct Tests

Certain Authorised Electrical Operators who:


a. have been authorised to and have carried out electrical operating procedures in the relevant work
area for a period of not less than 3 months, and

b. have been trained and satisfied the Train Electrical Manager or his representative as to their
knowledge and ability to safely and competently test apparatus
may be authorised by the Group Manager, Infrastructure Power to carry out those tests for which
they have been trained.

2.3 Levels of Electrical Operator Authorisation

Authorised Operators may be authorised in one or more work Areas. The areas and the levels of their
authorisation will be noted on their Authorisation Certificate.

Authorisations may be restricted to specific zones, areas, rail lines or apparatus at the request of the
Departmental Manager and approval of Group Manager, Infrastructure Power. Any restrictions will be
endorsed on the Authorised Electrical Operator Certificate before issue.

2.3.1 Traction Supply Work Area

Level T2 Authorised to operate, earth and short-circuit, issue and cancel Electrical
Access Permits on apparatus for which the Train Electrical Manager is
responsible.

Level T1 As for Level T2 as well as authorised to issue and cancel Testing Permits on
apparatus for which the Train Electrical Manager is responsible.

Issue, receive and relinquish Apparatus Switching Certificates or equivalent


from other Distribution Companies pertaining to the Traction Supply Work
Area.

2.3.2 Industrial Supply Work Area

Level I3 Authorised to operate, earth and short-circuit, issue and cancel Electrical
Access Permits and Permits to Work on HV outdoor Industrial Supply
apparatus (excluding fully enclosed industrial switchgear).

Level I2 As for level I3 as well as authorised to operate, earth and short-circuit, issue
and cancel Electrical Access Permits on indoor Industrial Supply apparatus and
fully enclosed industrial switchgear.

Level I1 As for level I2 as well as authorised to issue and cancel Testing Permits on all
Industrial Supply apparatus.

Issue, Receive and relinquish Apparatus Switching Certificates or equivalent


from other Distribution Companies covering the Corporation's Industrial
Supply apparatus.

2.3.3 Signal Supply Work Area

Level S3 Authorised to operate only on switches, circuit breakers and isolators of signal
supply apparatus which is part of the Signal Power Distribution System.

Level S2 Authorised to operate, earth and short-circuit, Issue and cancel Electrical
Access Permits and Permits to Work Near Electrical Apparatus on Signal
Apparatus which is part of the Signal Power Distribution System.

Level S1 As for level S2 as well as issue and cancel Testing Permits on Signal Supply
Apparatus.

Issue, receive and relinquish Apparatus Switching Certificates or equivalent


from other Distribution Companies pertaining to the Signals Work Area.

2.3.4 Overhead Lines Work Area

Level O2 Authorised to operate, test, earth and short-circuit 1500V conductors, DC


outdoor switches and isolators and issue Electrical Access Permits and Permits
to Work on 1500V traction apparatus.

Level O1 As for O2, as authorised to operate, test, earth and short-circuit High Voltage
AC outdoor switches and isolators and issue Permits to Work on HV
transmission lines.

Issue, receive and relinquish Apparatus Switching Certificates or equivalent to


other Distribution Companies pertaining to their responsible area.

2.3.5 Electrical Rolling Stock Area

Level ERS1 Authorised to operate siding earth switches or isolators, test and issue and
cancel Electrical Access Permits for work on the roof of Electrical Trains.

2.4 Competent Persons

2.4.1 Competent Persons are persons who are not less than 18 years of age and are in possession of a
current First Aid Level 1 certificate or equivalent and have been instructed and examined on
resuscitation and external cardiac massage as well as methods of release of victims from contact
with live apparatus. They are:

a. Competent and authorised to receive Electrical Access Permits because they understand:

(1) the meaning and significance of the Electrical Access Permits.

(2) the duties and responsibilities of the recipient of an Electrical Access Permit.

(3) the general nature of high and low voltage apparatus associated with their Work
Area.

(4) that where precautions provided in the Electrical Access Permit are in doubt,
clarification or additional protection shall be requested.

(5) that there are various ways apparatus may become live and unsafe.

and are:

b. Competent and authorised to assist Authorised Electrical Operators to carry out
defined operating procedures pertaining to their Work Area because they are
conversant with:

(1) apparatus approved for use in electrical operating.

(2) methodology of opening and closing circuit breakers, switches,


isolators etc.

(3) methodology of testing before earthing and short-circuiting electrical


apparatus.

2.4.2 Competent Persons shall be instructed and examined in the requirements of the position pertaining
to their work area and to the satisfaction of the Train Electrical Manager or his delegate, and be
certified by the Group Manager, Infrastructure Power.

The certification shall be reviewed at the discretion of the Group Manager, Infrastructure Power
and withdrawn when an employee no longer needs to be authorised or has demonstrated a loss or
lack of competency.

The form of Competent Person Certification is shown in Appendix "B".

2.4.3 Competent Persons do not normally perform electrical operations except for training purposes as
set out in Clause 5.1.5.

3. WORK IN THE VICINITY OF ELECTRICAL APPARATUS

3.1 Approval

Before any work is performed in the vicinity of electrical apparatus it shall be the responsibility of
the supervisor of the working party to obtain the approval of the Engineer-in-Charge of the
apparatus or nominee.

Before giving such approval, the Engineer-in-Charge of the apparatus shall arrange for adequate
safety precautions to be taken having regard to:

a. the nature of the work to be performed

b. the location of adjacent live electrical apparatus

c. whether or not the person is accustomed to working in the vicinity of electrical apparatus.

Where doubt exists as to who is the Engineer-in-Charge of the apparatus, enquiries are to be
directed to the Electrical Systems Officer.

3.2 Limit of Approach to Exposed Live Electrical Apparatus

No person shall allow any part of their body, or any object (approved devices excepted) in contact
with any part of their body, to come within two metres of any exposed live electrical apparatus
with exceptions as set out in Clause 4 and in the case of cranes, Appendix I.

FAILURE TO COMPLY MAY CAUSE


SERIOUS INJURY OR DEATH

The Engineer-in-Charge or nominee shall at their discretion, take all or any of the following
precautions:

a. Explain and point out, if applicable, the position of adjacent live electrical apparatus and
the limits of approach permitted.

b. Erect "Danger" notices to identify and define live electrical apparatus.

c. Erect barriers to guard against mistaken or inadvertent contact with electrical apparatus.

d. Appoint a suitable person as a safety observer whose sole duty shall be to warn employees
should the approach to live apparatus become hazardous.

e. De-energise adjacent live electrical apparatus.

4. WORKING ON ELECTRICAL APPARATUS
4.1 Limits of Approach to HV Electrical Apparatus

Authorised Electrical Operators, and Competent Person may approach exposed electrical apparatus to the
following absolute limits:

Voltage Range Absolute Limit


650 - 2000 volts (inclusive) 300mm

Above 2000 - 6000 volts (inclusive) 600mm

Above 6000 - 22000 volts (inclusive)- 1000mm

provided that they have satisfied the Engineer-in-Charge of the Apparatus or nominee that they are fully aware
of all electrical dangers in the particular work situation, or are working under the direct supervision of such a
person. However, the maximum practical clearance should be maintained at all times.

4.1.1 No person shall come closer to exposed electrical apparatus than the limit of approach set out above
unless the person is:

a. Working on apparatus which as been declared out-of-commission in accordance with


Clause 4.2, or

b. Applying or removing earthing conductors by hand in accordance with Clause 5.3, or

c. The recipient of an Electrical Access Permit or Testing Permit or a Permit to Work Near
Electrical Apparatus issued in accordance with these Rules, or

d. Working under the direct supervision of the recipient of an Electrical Access Permit, or

e. Working under the direct supervision of the recipient of a Testing Permit and is a member
of the same Testing Team, or

f. Working on live 1500 volt overhead apparatus in accordance with instructions issued by
the Manager Overhead Maintenance, or

g. Working on isolated and earthed and short-circuited AC or DC apparatus in accordance


with Clause 5 of these Rules, or

h. Working on live 2.2KV Signal apparatus with approved insulated devices in accordance
with instructions issued by either the Manager Metropolitan Signal Maintenance or the
Manager Country Signal Maintenance.

4.1.2 The time a person remains closer than one metre to exposed electrical apparatus must be restricted
to the absolute minimum consistent with the task being undertaken.

4.1.3 It is permissible to use approved stick operated disconnects on transmission lines with the line de-
energised but not isolated or earthed and short-circuited provided that the limit of approach of
Clause 3.2 is not infringed.

4.1.4 It is permissible to use an approved stick operated jumper to electrically connect two 1500 volt DC
overhead conductors. The device shall only be used when both conductors are believed to be live at
the same potential so that there is minimal opportunity for current flow between the two conductors
or both conductors are de-energised, and provided that the limits of approach in Clause 3.2 and
instructions issued for Live Line by the Manager Overhead Maintenance, are not infringed.

4.2 Out-of-Commission Apparatus

4.2.1 Electrical apparatus may be declared out of commission by the Engineer-in-Charge of the apparatus
or delegated representative and may be worked on without an Electrical Access Permit when it has
been disconnected from all sources of supply by the physical removal of a portion of each of the
conductors concerned, providing a break at least equal to that specified in Clause 4.1 or equivalent
as approved by the Manager Train Electrical.

Where the physical removal of a portion of each of the conductors is impracticable, the racking out
of a removable type circuit breaker shall be accepted as the "physical disconnection of the
conductors". In such cases, the instructions issued to declare the "out-of-Commission" shall be
endorsed by the Group Manager, Infrastructure Power. All means to replace the racked out circuit
breaker or gaining access to High Voltage terminals, shall be padlocked with a special lock, the keys

to which will be kept by the High Voltage Field Co-ordinator. Caution notices are to be fitted and
logged with Electrol.

The Engineer-in-Charge of the apparatus or delegated representative shall inform the Electrical
Systems Officer of the relevant details of the Out of Commission apparatus and the Electrical
Systems Officer shall record the information in the log book.

Where practicable, a sign or notice shall be affixed to the apparatus indicating that it is out of
commission, and include the signature of the Engineer-in-Charge and the date.

Notwithstanding the fact that the apparatus has been disconnected from all sources of supply, due
regard shall be given to the possibility of inadvertent energisation from adjacent apparatus by
induction, lightning or other means and where practicable, it shall be short-circuited and earthed.

4.2.2 A rack-out or removable type circuit breaker, switch or potential transformer may be considered to
be out of commission and may be worked on without an Electrical Access Permit after it has been
locked in the fully isolated position, or removed from the rack or completely withdrawn from its
cubicle and all covers locked over live or de-energised high and low voltage contacts.

4.2.3 If required, the Engineer-in-Charge of the apparatus may arrange for an Electrical Access Permit,
Switching Certificate or Permit to Work Near Apparatus to be issued on out of commission
apparatus to provide written confirmation that such apparatus is safe to approach. Such permits or
certificates shall be recorded by the Electrical Systems Officer.

4.3 Cable or Transmission Line Cut in Sections

4.3.1 When an electrical cable or transmission line is cut in two or more sections and it is intended
eventually to restore the line or cable to its original condition, a single Electrical Access Permit or
Testing Permit shall be taken out where only one party is to work on the line or cable.

In addition, precautions such as barriers, warning signs and safety observers shall be installed at the
cuts in the cable or line where necessary to ensure the safety of personnel and the public.

4.3.2 Where different parties are to work on separate sections of the cable or line, then each party shall
take out a separate Electrical Access Permit or Testing Permit for the section of the line or cable on
which they are to work and the Authorised Electrical Operator issuing the permit shall ensure that:

(a) The section covered by the Permit is clearly defined and has clearances at both ends of the
section of line or cable that are not less than those specified in Clause 3.2 or that suitable
approved barriers and warning signs are securely installed between the sections of the line
or cable.

(b) All other precautions such as barriers, warning signs and safety observers which may be
necessary to ensure the safety of personnel and the public have been taken.

(c) Separate earths are applied to each section of cable.

4.3.3 Any section of the cable or line which has been cut may be re-energised and recommissioned
provided that:

(a) No Electrical Permit (Access, Testing etc) or Certificate is on issue for that section.

(b) The ends of that section have the clearances specified in Clause 3.2 from any other section
on which an Electrical Permit or Certificate may be on issue.

4.3.4 When a cable or transmission line is cut for the purpose of joining to a different cable or line the
provisions of Clause 4.3.2 shall apply. In addition, Electrical Access Permits or a Testing Permit
shall be taken out on the cable or line to which the cut cable or line is to be joined.

5. SWITCHING, ISOLATING, EARTHING AND SHORT-CIRCUITING
ELECTRICAL APPARATUS

5.1 Persons Permitted to Perform Switching, Isolating, Earthing and Short-circuiting of Electrical
Apparatus

5.1.1 In an emergency involving an accident or serious risk of injury, employees who consider they
can do so with safety may open a circuit breaker, switch or isolator. They shall inform the
Electrical Systems Officer as soon as possible thereafter.

5.1.2 Any employee may operate a circuit breaker, switch or isolator for urgent occasions and only
when specifically instructed to do so by the Electrical Systems Officer.

5.1.3 To permit the loading and unloading of rail wagons in sidings equipped with 1500 volt overhead
apparatus, persons who have been approved by the Group Manager, Infrastructure Power, are
authorised to isolate and earth the overhead apparatus by means of an earthing isolator or switch.
Such an operation may only be performed with the permission of the Electrical Systems Officer.
A ‘Permit to Load or Unload Rail Wagons in Electrified Sidings’ may then be issued in
accordance with Section 6.4 of these Rules.

NOTE: Such isolating and earthing of 1500 volt sidings shall be regarded as
precautionary only and shall NOT permit any person or unapproved object to
approach to within 300 millimetres of the overhead which shall at all times be
regarded as live.

5.1.4 Corporation employees from groups other than Infrastructure Power Group and employees of
contractors to the Corporation who have been instructed in the operation of 1500 volt isobars or
switches to the satisfaction of the Group Manager, Infrastructure Power, are authorised to
operate earthing isolators or switches in the following locations:
Train Light Repair Centres
Train Maintenance Depots
Newport Workshops, and
Electrified Sidings
to enable them to work safely on rolling stock.

The permission of the Electrical Systems Officer must be obtained before operating any such
isolator or switch, except that the permission of the Electrical Systems Officer is not required to
operate switches or isolators which only control the supply to 1500 volt wiring or apparatus that
is erected solely within workshops building.

The control of the operation of switches or isolators for 1500 volt wiring or apparatus within
these buildings is the responsibility of the Manager of that building.

5.1.5 For the purpose of training only and with the prior permission of the Electrical Systems
Officer, persons acting under the direct instruction and on-site supervision of an Authorised
Electrical Operator are permitted to perform switching, isolating, earthing and short-circuiting
including the application of stick and hand earths and short-circuits as well as issuing Electrical
Access and Testing Permits.

5.1.65 Authorised Electrical Operators shall only perform switching, isolating, earthing and short-
circuiting on apparatus for which their training and authorisation is currently appropriate.

5.1.7 An Authorised Electrical Operator operating alone may perform any switching and isolating
of electrical apparatus except for the application of hand earths and hard short-circuit
devices.

5.2 Operation of Circuit Breakers, Switches and Isolators

5.2.1 Approval of the Electrical System officer is required except as detailed in Clause 5.1.1.
No circuit breaker, switch or isolator shall be closed without the permission of the
Electrical Systems Officer.

No circuit breaker, switch or isolator shall be opened without the permission of the Electrical
Systems Officer except in an emergency involving an accident or serious risk of injury.

All switching operations shall be reported to the Electrical Systems Officer as soon as possible
after each operation unless permission has been given by the Electrical Systems Officer to carry
out specific operational checks on defined apparatus.

Severe arcing, or apparent damage to isolators, switches or circuit breakers shall be immediately
reported to the Electrical Systems Officer.

5.2.2 Operation of circuit breakers and switches shall be as follows:

Only the approved handle or mechanism shall be used for manually operating a circuit breaker or
switch.

The manual operation of a circuit breaker or switch should be performed in an unhesitating and
controlled manner.

After opening or closing a circuit breaker or switch, its correct operation shall be confirmed by
inspection of the circuit breaker or switch itself if this is practicable, or by observation of the
indicating devices or lamps which are provided for the purpose.

If the power supply to these indicating lamps or devices has been removed or is unavailable and
the power supply becomes restored, the condition of the lamps and devices must be checked
immediately to confirm the condition of the circuit breaker or switch.

For the signalling supply distribution system, in addition to the above requirements the correct
operation shall be confirmed by the observation of the energised (or de-energised) state of the
signalling equipment associated with that signal supply.

5.2.3 Operation of Isolators

An isolator shall be opened only after it has been ascertained that the circuit has been opened
elsewhere by means of a circuit breaker or switch and the isolator shall be closed before the
circuit breaker or switch is closed.

Notwithstanding the foregoing, an isolator may be used to make or break the excitation current
of transformers, the charging current of transmission lines and underground cables, and DC
circuit breaker control current when, in the opinion of the Electrical Systems Officer, it is safe to
do so.

Only the approved handle, mechanism or operating stick shall be used for operating isolators.

Where practicable, the correct operation of the isolators shall be confirmed by visual inspection.

When opening an isolator by means of an operating stick, the operator shall be in control of the
isolator and the isolator is to be reclosed immediately should severe arcing occur across the open
contacts.

Where possible, the line isolators for DC circuit breakers shall be opened before the bus or zone
isolators.

After closing a latching type isolator, a check must be made to ensure the isolator is effectively
latched in.

In the case of ‘rack out’ type equipment, isolation (from the bus, circuit or feeder) shall be
carried out by the equipment being placed and locked in the isolated position. The contact covers
shall then be locked closed and shall not be unlocked or opened except for special purposes such
as testing and/or earthing and short-circuiting.

5.2.4 Co-ordinated Operation of Switches and Isolators

Where two or more Authorised Electrical Operators are performing switching and/or isolating
apparatus which is electrically interconnected, the Electrical Systems Officer

shall co-ordinate the operation.

5.2.5 Person to Communicate with Electrol

The operator who is actually undertaking the operating is the person to be in communication with
Electrol unless prior approval has been obtained from the responsible Electrical Systems Officer.

5.3 Earthing and Short-circuiting Electrical Apparatus

5.3.1 Only approved earthing and short-circuiting devices shall be used to earth and short-circuit
electrical apparatus.

5.3.2 Before using an earthing and short-circuiting device, it shall be examined to ensure that it is
in good order.

5.3.3 Before attempting to earth and short-circuit electrical apparatus, consideration shall be given
to whether or not the earthing and short-circuiting devices can be applied with safety, and
application of the devices shall only be attempted when the design of the earthing device and the
apparatus permits the work to be done safely.

5.3.4 an earthing and short-circuiting device shall be applied only after the electrical apparatus has
been isolated and only after confirmation with an approved live line detector that the apparatus is
de-energised at the point at which the earth and short-circuit is to be applied, except where this is
precluded by the design of the apparatus.

The operation of the live line detector shall be checked immediately before the test and, if
practicable, immediately after the test.

5.3.5 When applying an earthing and short-circuiting device, it shall be connected to earth before
being connected to the electrical apparatus and it shall be disconnected from the apparatus before
being disconnected from earth.

5.3.6 Because of the possibility of apparatus not being de-energised at the moment of application
of the earthing or short-circuiting device, the apparatus shall be regarded as liable to become live
until the earth and short-circuit has been completed.

Therefore, during application of earthing and short-circuiting devices, other than hand earths and
hand short-circuits, the earthing and short-circuiting conductors shall not be touched by any
person.

5.3.7 Whenever practicable, earthing and short-circuiting devices shall be applied to and removed
from exposed apparatus by means of the sticks approved for the purpose.

However, where the application and removal of earths and short-circuits by hand is unavoidable,
the apparatus shall be discharged by means of a short-circuiting and earthing device applied by
an operating stick or approved device which shall be maintained in contact with each of the
conductors in turn while the hand earth and short-circuits are being applied or removed.

5.3.8 An Authorised Electrical Operator shall be assisted by at least a Competent Person when
applying hand earths and short-circuits.

5.3.9 A competent Person under the direct supervision of an Authorised Electrical Operator may
apply and remove earths and short-circuits for the purpose of training only and with the prior
permission of the Electrical Systems Officer.

5.3.10 When an earth and short-circuit is applied or removed by means of an isolator or switch
which can be moved to or from the ‘earth’ position, the isolator or switch shall be inspected
where practicable to confirm that it has operated correctly.

5.3.11 When an earth or short-circuit is applied by means of an isolator, switch or circuit breaker
which cannot be visually inspected, the earthing or short-circuit shall not be regarded as effective
until a visible earthing or short-circuiting device has been applied, except where the design of the
apparatus makes this impossible.

5.3.12 When electrical apparatus is being earthed to earth spikes, power rails or overhead

structures in accordance with 5.3.13 and 5/3/14 below, the Authorised Electrical Operator shall
ensure that other persons do not approach closer than six metres to the earth spikes in the
vicinity of the earth clamp, rails or overhead structures until the earthing circuit has been
completed.

5.3.13 1500V DC conductors should be earthed by connection to the power rail or negatives in
substation areas. In exceptional cases where this is not feasible, 1500V DC conductors supported
on steel structures may be earthed by connecting to the steel structure, provided it is established
that:
(a) the steel structure is connected to the power rail;
or
(b) the steel structure is connected by aerial conductor to another structure earthed
as in (a) above.

5.3.14 In the case of steel masts not carrying 1500V conductors, there will be no spark gap but the
structure may be earthed either to an earth spike or mat or by a cable or aerial conductor to
another structure. In each of these cases it is necessary only to earth the apparatus to the structure
after verifying that those conditions exist.

5.3.15 When electrical apparatus is being earthed to the traction rail, great care must be taken to
ensure that the rail to which the earth is connected has been bonded for traction power and is not
a signal track circuit rail only.

5.3.16 Before any electrical insulated cable is cut for the purpose of cable jointing, the cable shall
be earthed and short-circuited at the worksite by an approved, remotely controlled earthing and
short-circuiting device. Such device shall be used only after the cable has been earthed and short-
circuited at both ends and an Electrical Access Permit has been issued and only after the utmost
care has been taken to identify the correct cable.

After the remote controlled earthing has been applied and before commencement of work on the
cable, the authorised operator must confirm with the Electrical Systems Officer that no other HV
circuits in the immediate work area have had a circuit breaker opening.

5.3.17 No earth or short-circuit which is specified on a Permit or Certificate shall be removed


without the permission of the Electrical Systems Officer.

In the case of Testing Permits, the tester must indicate the circumstances of the test and receive
the Electrical Systems Officer’s approval prior to conducting the Test. Permission for removal of
each earth and short circuit during the Test is not required.

5.3.18 Earths and short-circuits shall be applied on the apparatus between the worksite and all
sources of supply. In the case of line work, earths should be as close to the worksite as
practicable.

5.3.19 When sidings are earthed via an earth switch or isolator for the purpose of working on the
roof of Electric Rolling Stock, the de-energisation of the 1500V siding overhead apparatus shall
be checked using an approved live line detector before issuing an Electrical Access Permit. A
permit issued under those conditions allows for work on the roof of trains only but does not
allow for contact with the overhead.

6. ELECTRICAL PERMITS, CERTIFICATES AND EQUIVALENTS

6.1 Apparatus Switching Certificates

When persons of any other organisation are required to work on or near electrical apparatus under the
control of the Corporation, an Apparatus Switching Certificate shall be issued by the Corporation’s
Authorised Electrical Operator to that organisation specifying the conditions of switching, isolating,
earthing and short-circuiting of the particular apparatus and certifying that the conditions of switching and
isolating will remain unchanged by the Corporation until the certificate is cancelled.

Earths may be removed by the issuing Network Operator only at the request of the recipient and notification
to the Electrical System Officer. The Recipient may energise the apparatus with test voltages after
notifying the Electrical Systems Officer.

The Operator of the recipient organisation may inspect and verify the security and adequacy of the
precautions taken by the Corporation’s Authorised Electrical Operator.

6.1.1 Apparatus Switching Certificates shall be issued and cancelled only by Authorised Electrical
Operators with the permission of the Electrical Systems Officer.

6.1.2 Apparatus Switching Certificates, Statement of Condition of Apparatus and/or Plant Certificates
or equivalents shall be received and relinquished only by Authorised Electrical Operators.

6.1.3 A Competent Person under the direct supervision of an Authorised Electrical Operator may issue
and cancel an Apparatus Switching Certificate for the purpose of training only and with the prior
permission of the Electrical Systems Officer.

6.1.4 When issuing an Apparatus Switching Certificate, the apparatus shall be earthed and short-
circuited if requested by the receiving organisation, except that where the Corporation considers
this to be undesirable, other precautions such as short-circuiting only shall be taken and the
absence of earths shall be recorded on the Apparatus Switching Certificates.

6.1.5 Where apparatus entirely controlled by the Corporation has been isolated and earthed and short-
circuited for the purpose of issuing an Apparatus Switching Certificate to another organisation,
an Electrical Access Permit may be issued by the Corporation’s staff to perform work on the
same apparatus providing that the work does not alter the conditions of isolating and earthing
and short-circuiting specified on the Apparatus Switching Certificate and provided that the
organisation’s recipient of the Apparatus Switching Certificate has been notified of the intention
to issue the Electrical Access Permit.

6.1.6 Where apparatus jointly controlled by the Corporation and another organisation has been
isolated and earthed and short-circuited for the purpose of issuing an Apparatus Switching
Certificate to that organisation, an Electrical Access Permit may be issued to the Corporation’s
staff to perform work on the same apparatus providing that a Statement of Condition of
Apparatus and/or Plant or equivalent has been received from the other organisation stating the
switching, isolating, short-circuiting and earthing will remain unchanged by that organisation
until the Statement is cancelled.

6.1.7 Apparatus Switching Certificates shall only be used between the Corporation and another
organisation but not within the train infrastructure electrical network except for training purposes
as detailed in Clause 6.1.3.

6.2 Electrical Access Permits

An Electrical Access Permit is an authorisation issued to allow access to, and work upon, isolated and
earthed rail traction voltage apparatus; or isolated earthed and short circuited HV apparatus.

6.2.1 Electrical Access Permits shall be issued and cancelled only by Authorised Electrical Operators
and only with the permission of the Electrical Systems Officer.

6.2.2 Electrical Access Permits shall be received and relinquished only by Competent Persons
and cannot be transferred to another person.

6.2.3 Authorised Electrical Operators may issue an Electrical Access Permit to themselves provided
they are accompanied by a Competent Person. In such an event, the Competent Person shall
sign the Permit as a member of the ‘working party’.

6.2.4 A trainee under the direct supervision of an Authorised Electrical Operator may issue and cancel
an Electrical Access Permit only:

(a) For the purpose of training and with the prior permission of the Electrical Systems
Officer.

(b) For planned work with the co-signature of the Authorised Electrical Operator and with
the prior permission of the Electrical Systems Officer.

6.2.5 On receipt of an Electrical Access Permit, the recipient in charge must ensure that all persons
who require access to the apparatus sign the permit form before commencing work.

6.2.6 An Electrical Access Permit shall not be relinquished and cancelled until all persons who have
signed on the permit have signed off. (Refer 6.2.7 for exceptional circumstances.)

6.2.7 If due to exceptional circumstances, it is impossible for a member of the work party to sign off
the permit, the following procedure must be adopted :

(a) The recipient in charge shall ensure that adequate precautions are taken to advise and
prevent such a person from approaching the apparatus.

(b) The recipient in charge shall advise the Electrical Systems Officer that a member of the
work party is unable to sign off the permit and that adequate precautions have been
taken to prevent such a person from approaching the apparatus.

(c) The Electrical Systems Officer shall log in the log books that attempts have been made
to locate the missing member of the work party and only then, with the permission of
the Electrical Systems Officer, may the recipient in charge sign off instead of the
missing person.

6.2.8 An Electrical Access Permit and an Testing Permit shall not be on issue on the same apparatus at
the same time.

6.2.9 More than one Electrical Access Permit may be on issue on the same apparatus at the same time
provided that separate earths and short-circuits are applied for each Electrical Access Permit,
except where the Electrical Systems Officer deems this to be impractical. In such cases it shall
be the responsibility of the Electrical Systems Officer to co-ordinate the cancellation of
Electrical Access Permits, the removal of earths and short-circuits and the closing of isolators so
that the actions of one party shall not endanger the safety of others.

6.2. 10 A Competent Person or Authorised Electrical Operator must be in possession of an Electrical


Access Permit if they intend to work on electrical apparatus or within the limits of approach to
such apparatus permitted in Clause 4.

6.2.11 An Electrical Access Permit, except at the discretion of the Electrical Systems Officer, shall not
remain issued for a period greater than 24 hours.

6.3. Testing Permit

6.3.1 A Testing Permit is the form of authorisation issued and cancelled with the approval of the
Electrical Systems Officer where any form of testing of electrical apparatus is required which
necessitates :

(a) Removal of any earths or short-circuits, or

(b) Energisation either by normal sources or test supplies; or

(c) Approach to high voltage apparatus within the limits specified in Clause 4; or

(d) Attachment of any test apparatus to electrical apparatus.

6.3.2. The electrical apparatus shall be isolated and earthed or short circuited for the issue of a Testing
Permit, except where the person responsible for testing indicates that it is essential for the
apparatus to be live. In these circumstances the prior approval of the Electrical Systems Officer
is necessary and the existing condition of the apparatus shall be stated on the Testing Permit
form.

However, where during the course of testing it is necessary for any person to come into contact with
portions of the apparatus which may be live or to approach the apparatus with devices other than
those approved for the purpose of working on live apparatus, then in each and every case the
apparatus shall be isolated, tested, earthed and short-circuited before any contact is attempted.

6.3.3 Electrical apparatus under test shall not be left unattended in a condition where it is not
earthed and short-circuited unless adequate precautions have been taken, e.g. notices are
displayed and barriers are erected to draw attention to the unusual condition, and then only with
the permission of the Electrical Systems Officer.

6.3.4 Testing Permits shall in all cases, with prior permission of the Electrical Systems Officer,
be issued by appropriately Authorised Electrical Operators to themselves and cancelled by
themselves provided they are accompanied by a Competent Person with the exception as in
Clause 6.3.6 below.

6.3.5 Where necessary and with prior permission of the Electrical Systems Officer, a Testing
Permit may be transferred to a second appropriately Authorised Electrical Operator who shall
assume all responsibility for safety, testing and the cancellation of the Testing Permit.

6.3.6 An Electrical Operator authorised to a level below TI, I1 or S1 as applicable, and under the
direct supervision of an appropriately Authorised Electrical Operator may issue a Testing Permit
for the purpose of training only and with the prior permission of the Electrical Systems Officer.

6.3.7 It shall not be permissible to issue a Testing Permit in cases where the work may be efficiently
and properly performed under an Electrical Access Permit.

6.3.8 A Testing Permit and an Electrical Access Permit shall not be on issue on the same
apparatus at the same time, nor shall more than one Testing Permit be on issue on the same
apparatus at the same time.

6.3.9 Any electrical test involving the removal of earthing or short-circuiting devices on the
apparatus must be carried out under the protection of an Testing Permit in accordance with these
Rules. Live line detectors are not included as they are used for testing whether apparatus is live
or not.

6.4 Removed

6.5 Permit to Work

6.5.1 Permit to Work Certificates shall be issued and cancelled only by Authorised Electrical
Operators with the permission of the Electrical Systems Officer.

6.5.2 When it is necessary for a person to work in the vicinity of but not in contact High Voltage
electrical apparatus, the responsible manager of the electrical apparatus or his appointed
representative shall determine whether the work may be safely undertaken with the apparatus
live.

6.5.3 If in the opinion of the responsible manager of the electrical apparatus or the appointed
representative the work cannot be carried out with the apparatus live, the responsible manager of
the electrical apparatus shall then arrange for the apparatus to be isolated and earthed and a
Permit to Work issued.

6.5.4 Arrangements for the issue of a Permit to Work shall only be made on the receipt of an
Application to Work in the Vicinity of Electrical Apparatus.

6.5.5 A Permit to Work may be transferred up to 5 times with the approval of the Electrical Systems
Officer.

6.5.6 A Permit to Work may not be on issue greater than 3 days. In such an event, the Permit to Work
must be cancelled and a new Permit to Work issued or the apparatus taken Out of Commission in
accordance with clause 4.2.

6.5.7 Persons who have been authorised to work near the electrical apparatus under their own isolation
and protection are exempt from the need to hold a Permit to Work, but must inform the
Electrical Systems Officer.

6.5.8 A Permit to Work shall only be issued to and received by a person who has been trained,
examined and authorised to do so.

6.5.9 In the event that the Permit to Work is lost, the following steps must be taken:

(a) The recipient will ensure that all work covered under the PTW ceases and all members of
the work party informed until a new PTW can be issued

(b) The recipient will notify the Electrical Systems Officer

(c) The Electrical Systems Officer will then arrange for the cancellation of the Permit to
Work that is on issue and/or arrange for a new Permit to Work to be issued.

6.6 Statement of Condition of Apparatus and/or Plant Certificates or Equivalent

6.6.1 When the Corporation’s staff are required to perform tests or work on or near electrical
apparatus under the control of another Network Operator, the Corporation’s Authorised
Electrical Operator shall, BEFORE ISSUING AN ELECTRICAL ACCESS OR TESTING
PERMIT, receive from the other Network Operator a Statement of Condition of Apparatus
and/or Plant Certificate or similar certificate.

Such certificates are to state the conditions of switching, isolating, short-circuiting and earthing
and will certify that the conditions of switching and isolating will remain unchanged by the
issuing Authority until the certificate is cancelled.

6.6.2 Where a Testing Permit has been issued by the Corporation’s Authorised Electrical Operator
on apparatus covered by the certificate, the earths and short circuits as stated on the certificate
may be removed by the issuing Authority only at the request of the recipient of the certificate and
only if permission is granted by the Electrical Systems Officer. Removal of earths must be then
noted on the Statement of Condition of Apparatus Certificate.

The apparatus may he energised with test voltages only if permission is granted by the other
Network Operator and the Electrical System Officer.

6.6.3 Before receiving a Statement of Condition of Apparatus and/or Plant Certificate or equivalent,
the Corporation’s Authorised Electrical Operator may inspect and verify the security and
adequacy of the precautions taken by the other Network Operator.

NOTE: A STATEMENT OF CONDITION OF APPARATUS AND/OR PLANT


CERTIFICATE ALONE IS NOT AN AUTHORITY TO PERFORM
WORK ON THE APPARATUS.

6.6.4 Only appropriately Authorised Electrical Operators shall receive and relinquish Apparatus
Switching Certificates, Statement of Condition of Apparatus and/or Plant Certificates or
equivalents issued by another Supply Authority and only with the permission of the Electrical
Systems Officer.

7. MAKING ELECTRICAL APPARATUS LIVE

7.1 Electrical apparatus shall not be placed in service except on the instruction of the Engineer in
Charge of the apparatus and with the permission of the Electrical Systems Officer.

7.2 The Electrical Systems Officer shall first ascertain that no Electrical Certificates or Permits are on
issue and that all earths and short-circuits have been removed.

7.3 Before any connection is made whereby new apparatus or apparatus previously out-of-commission
could be made live by normal switching from an existing power supply system the Electrical
Systems Officer shall be notified and shall thereafter exercise full control over the operation of the
apparatus under the terms of these Rules. Such connection and any subsequent work on the
apparatus shall be performed under an Electrical Access Permit or Testing Permit as appropriate.

7.4 New apparatus previously out of commission shall not be placed in service until appropriate tests as
required by the Engineer in Charge and approved by the Group Manager, Infrastructure Power have
been completed and the test results have been agreed by the Engineer in Charge of the apparatus
and approved by the Group Manager, Infrastructure Power.

7.5 Apparatus out of service for repairs shall not be placed in service until appropriate tests have been
successfully completed to meet the Corporation's accepted standards for such apparatus. These
standards are set by the Engineer in Charge of the apparatus to the satisfaction of the Group
Manager, Infrastructure Power.

7.6 Prior to placement of any previously out-of-commission or new electrical equipment into service,
the Engineer in charge of that equipment shall issue a notice to the relevant Work Areas to advise
that on the given date, that equipment will be placed in service and is to be considered live. It is the
responsibility of each Work Area to relay the information to relevant staff.

8. GENERAL INSTRUCTIONS

8.1 Accuracy of Instructions

All communications whether written or verbal relating to the operation of electrical apparatus shall
be clear and definite.

Apparatus shall be referred to by its correct designation.

To avoid misunderstanding, the recipient of verbal instructions shall repeat back the instructions
and receive confirmation that the repetition is correct.

8.2 Security

8.2.1 Circuit breakers, switches and isolators which are inaccessible to the public or to
unauthorised employees shall not be locked in the "closed" position.

8.2.2 Circuit breakers, switches, and isolators which are accessible to the public or to
unauthorised employees shall be locked in the "open" or "closed" position by means of
padlocks.

8.2.3 Circuit breakers, switches and isolators which are operated for the purpose of issuing
Electrical Permits or Certificates shall where practicable be locked in the appropriate
position by means of safety padlocks with keys to differ.

Where practicable, such keys shall be held by the recipient in charge until the Permit or
Certificate has been relinquished and cancelled, or, the security of the keys shall be
determined by the Electrical System Officer.

Where other Network Operators have used their safety padlock on the Corporation's
apparatus, they shall be responsible for the security of their own keys.

Where padlocking of circuit breakers, switches and isolators which are operated for the
purpose of issuing electrical permits or certificates is not practicable, "CAUTION" notices
shall be displayed on that equipment during the currency of the permit or certificate.

8.2.4 Persons shall not enter a substation, tiestation, switchroom, switchyard or other enclosure
containing electrical apparatus except in the course of their duties.

8.2.5 Employees who have a need shall, immediately on entering a substation, tie station or
switch room, or, before entering a switch-yard or other enclosure containing high voltage
apparatus, advise the Electrical Systems Officer of their presence and the nature of the
work they intend to perform. Their date and time of entry, the task to be performed, and
their time of departure shall be recorded in the locations log book.

8.3 Live Line Detectors

An approved Live Line Detector shall be used before applying an earth and short-circuit device to
electrical apparatus except where the design of such equipment makes such tests impracticable. A
testing permit is not required for the use of a live line detector.

In all cases, live line detectors shall be used strictly in accordance with the instructions
accompanying the instrument.

Live Line Detectors may be divided into two classes:

(a) Those requiring direct contact with conductors for correct operation.

(b) Those requiring only proximity to conductors for correct operation.

In the case of detectors requiring direct contact, it is essential that the live line detector be
connected to the negative, power rail, earth or neutral conductor before being applied to the positive
or active conductor. This is not the case for two phase signal supplies when detecting across two
phases.

Where possible Live Line Detectors shall be checked for correct operation by application to a
known live conductor or by use of the checking facility internal or external to the detector, both
before and after application to the apparatus to be checked.

8.4 Operating Sticks

8.4.1 Only operating sticks that have been approved by the Group Manager, Infrastructure
Power adequately cared for and tested shall be used for operating electrical apparatus. It is
the responsibility of each manager to ensure compliance with these conditions.

8.4.2 Operating sticks shall be subjected to appropriate tests at least every 5 years or more
frequently in accordance with Departmental or company instructions.

8.4.3 Each operating stick shall be appropriately marked with an identifying number, the
maximum voltage on which the stick may be used and the date of the next routine test.

8.4.4 Operating sticks shall be stored out of the weather, kept clean, in good condition and not
placed on the ground.

Operating sticks shall be inspected by the operator prior to use to determine their serviceability.
Any defects are to be promptly reported to the Supervisor. Defective sticks are NOT to be used

8.4.5 Care shall be exercised during use to ensure that:

(a) the effective insulation of the operating stick shall not be reduced by allowing it
to make contact with intermediate conductors or earth; and

(b) the operating hook of a stick is not allowed to reduce air gaps of switches or
isolators (possibility of flash over).

8.4.6 Authorised Electrical operators should avoid the use of operating sticks in the operation of
electrical apparatus in wet weather conditions.

8.5 Earthing and Short-circuiting Device

8.5.1 Only earthing and short-circuiting devices that have been approved by the Group
Manager, Infrastructure Power may be used for earthing and short circuiting of electrical
apparatus.

8.5.2 These devices must be inspected by the operator prior to use to determine their
serviceability. Any defects are to be promptly reported to the Supervisor. Defective
devices are NOT to be used.

WARNING

ALL ELECTRICAL APPARATUS UNLESS ISOLATED AND EARTHED, MUST BE TREATED AS LIVE
AND WORK ON OR IN THE VICINITY OF THE APPARATUS SHALL BE CARRIED OUT ACCORDING
TO THESE ELECTRICAL SAFETY RULES. FAILURE TO PROCEED IN THE FOLLOWING WAY MAY
CAUSE DEATH.

9. LIVE LINE WORKING ON HIGH VOLTAGE APPARATUS

9.1 Apparatus on which Live Line Work can be carried out.

Live Line Work may be carried out on 1500 volts apparatus of the Overhead Electric Traction System and in
accordance with the instructions issued by the Overhead Maintenance Manager and Group Manager,
Infrastructure Power.

9.2 Staff Permitted to do Live Line Work

Live Line Work shall only be undertaken by employees of the Overhead Department who have been trained,
examined and passed in live line work techniques for their work area.

9.3 Circumstances Permitting Live Line Work

Live Line Work shall be permitted only when it is impractical to do the work with the apparatus isolated and
earthed. The Manager of an area has to ensure that this is the case.

When circumstances prevent the apparatus being isolated and earthed, live line work shall only be done if it
can be performed safely.

The employee in charge at the site of the work shall determine if the work can be performed safely. However,
each employee required to work live line must then be satisfied that the work can be performed safely and in
accordance with their work area instructions for live line working.

9.4 Working Platforms for Live Line Work

Live Line Work shall only be carried out from a suitable approved working platform.

Such working platforms include :-

(a) Approved wooden extension ladders without wire binding.

(b) Other approved Apparatus.

9.5 Clearance when working Live Line

9.5.1 In all cases except those in Clause 4 employees working Live Line shall not approach with tools or
equipment, clothing or any part of the body to :

(a) within 1200 millimetres of dead or earthed apparatus

(b) within 1200 millimetres of live apparatus of a different circuit or section, unless he is
applying an approved short circuiting device between the two sections with the
permission of the Electrical Systems Officer.

The following precautions shall be taken to ensure clearance is maintained.

9.5.1.1 No metallic or conducting object shall be passed by hand between an employee on the
ground and any employee working live line on live high voltage apparatus.

9.5.1.2 No employee shall bridge with tools, clothing, body etc., any insulator, air gap, section
insulator or insulated knuckle.

9.5.1.3 No employee shall bridge with tools, clothing, body etc., between one live circuit or
section and another live circuit or section or between one live circuit or section and a
neutral circuit or section.

9.5.2 Approach to dead apparatus or live apparatus of a different circuit or section by an employee
working live line may be less than 1200 millimetres if any or all of the following precautions are
taken:

9.5.2.1 Point out the adjacent apparatus and appoint a safety observer person whose sole
duty is to warn the employee of the approach to the dead apparatus or different
live section.

9.5.2.2 Erect an approved safety appliance or barrier to prevent contact with the dead
apparatus or different live section.

9.5.2.3 With the permission of the Electrical Systems Officer provide a suitable
approved shorting conductor between the live conductors of the different circuits
or sections.

Operation Of Vehicles And Mobile Plant
In The Vicinity Of Exposed Live Electrical Apparatus
1. Scope

This Appendix describes the rules which shall apply to all mobile plant and vehicles working in the vicinity of
exposed live electrical apparatus including 1500 Volt DC and 22 KVAC apparatus such as traction distribution
overhead lines. This Appendix is supplementary to section 3.0 of the Rules.

2. Definitions

The following definitions shall be used for the purposes of this Appendix. Other definitions are given in
Section 2.0 of the Rules.

Limit of Approach: The minimum distances which shall apply to employees, contractors, vehicles and mobile
plant, including elevating work platforms approaching exposed live electrical apparatus (See Section 3).

Mobile Plant: Cranes, elevating work platforms, tip trucks or similar plant, any equipment fitted with a jib or
boom and any device capable of raising or lowering a load. When determining limits of approach, Mobile
Plant can only be considered as a vehicle when in the normal travelling mode and not in the working mode.

Vehicle: A truck (not tipping), car, utility or other general purpose vehicle used for the carriage of personnel,
or goods (see Mobile Plant).

3. Vehicles Used To Carry Out Work In The Vicinity Of Exposed Live Electrical Apparatus

A person in charge of any vehicle used to carry out work in the vicinity of exposed live electrical apparatus
including overhead lines, except when operating as mobile plant, shall ensure that no part of its vehicle or load
is placed or moved within 2.0 metres of exposed live electrical apparatus allowing for the most adverse
conditions of sag and sway. To maintain these minimum distances appropriate allowance shall be made for
uncontrolled movement of the vehicle due to any reason.

4. Departmental Procedures

Exemptions to this Appendix may apply as covered by specific Departmental Procedures. The procedures shall
be approved by the Engineer in Charge of the Apparatus and authorised by the Group Manager Infrastructure
Power.

5. Operation Of Mobile Plant

5.1 Before any mobile plant is brought to a site where there are overhead power lines or electrical
apparatus and again before the mobile plant is set in position on the site, the person in charge of the
mobile plant and the supervisor in charge of works must make a thorough examination of the site and
its approaches in order to determine whether there is a need for taking of special precautions against
electrical hazards which may result from operation of the mobile plant.

5.2 The person in charge of the mobile plant shall, through all employees and contractors engaged in the
work, ensure that the mobile plant, its gear and load shall not approach within 6.4 metres of exposed
live electrical apparatus including overhead lines, allowing for the most adverse conditions of sag and
sway unless:

• given specific approval by the Engineer in Charge in line with section I (5.3); or

• the electrical apparatus is isolated and earthed in accordance with these rules.

The Engineer in Charge may increase the Limit of Approach if circumstances warrant.

Where mobile plant is used on any site in the vicinity of exposed live electrical apparatus or where it
may infringe the Limit of Approach of exposed live electrical apparatus, consideration shall be given
by the Engineer in Charge to the isolation and earthing of the apparatus or its removal from site for the
duration of works. Where it is not practical to do so the following shall apply.

5.3 Operation of Mobile Plant its gear or load between 6.4 metres and 2.0 metres of exposed live
electrical apparatus including overhead lines, allowing for the most adverse conditions of sag and
sway may be approved by the Engineer in Charge subject to all of the following been carried out.

(a) The Engineer in Charge of the electric apparatus must be advised whenever mobile plant is to
be operated within the above limit of approach or when in overturning, it would come into
contact with that electrical apparatus.
(b) The Engineer in Charge must be advised of the situation at least 48 hours or more (depending
on the planning required) prior to commencement of operations to enable site inspection and
to ensure that the situation is properly controlled.
(c) The Engineer in Charge shall nominate a responsible person to observe at all times the
clearance between the mobile plant, its gear and load and the exposed live electrical
apparatus. That person (the safety observer) shall be instructed to signal the operator to stop
whenever that limit of approach appears to be less than 2 metres or the approach becomes
hazardous allowing for the most adverse conditions of sag and sway.
(d) The safety observer shall not be given any task other than observing the limit of approach
between the mobile plant and the exposed live electrical apparatus.
(e) The Engineer in charge must advise the Electrical Systems Officer of precautions taken on the
worksite, along with operation times, names and telephone numbers of persons involved in
the working of the mobile plant.
(f) The safety observer shall advise the Electrical Systems Officer immediately before work
starts and after work finishes.
(g) The Electrical Systems Officer must arrange for the operating points of the electrical
apparatus to be tagged or otherwise marked on the SCADA system display until advice is
received that all mobile operations are completed.
(h) In the event of any automatic opening of circuit breakers supplying the power lines or 1500
volt sections in the vicinity of a worksite, the Electrical Systems Officer shall not re-close the
circuit breakers until he has made inquiries with the worksite and has satisfied himself that it
is safe to re-close the circuit breakers.
(i) Two means of communication shall be available to the safety observer on site, eg. pager and
telephone.
(j) The use of approved wooden ladders and insulating gear (eg. pullout sticks rated for 22 kV
AC for voltages up to and including 22 kV AC) is mandatory when working in the vicinity of
electrical apparatus.
(k) The Engineer in Charge of the electrical apparatus may also require additional precautions to
be taken such as posting of additional lookout men, connection of a trailing cable from the
crane to an earth or spark-gapped structure, erection of safety barriers and/or notices or fitting
an approved device to restrain the jib, boom or part of the mobile plant.

5.4 When the mobile plant its gear or load will infringe or is likely to infringe to within 2.0 metres of the
exposed live electrical apparatus including overhead lines, allowing for the most adverse conditions of
sag and way, then work in these circumstances can only be carried out under isolated and earthed
conditions and after issue of a “Permit to Work (Form 526)”. The Engineer in Charge of the overhead
electrical apparatus must be informed by the person in charge of the mobile plant operations at least
14 days in advance of works.

A “Permit to Work” may be issued upon receipt by the Engineer in Charge of the electrical apparatus
of an “Application for a Permit to Work in the Vicinity of Overhead Equipment”.

The Engineer in Charge shall arrange with the Electrical Systems Officer for the necessary switching,
isolating and earthing prior to issue of the “Permit to Work” to the person in charge of the mobile
plant operations.

The mobile plant operator must observe the conditions of the Permit to the Engineer in Charge on
completion of the mobile plant operations.

When more than one mobile plant is used at a worksite, only one “Permit to Work” may be issued.
Each mobile plant operator shall observe the conditions of the Permit and have those conditions noted
in his diary. The Permit shall be retained by the person in charge of the worksite.

In addition to the protection offered by the “Permit to Work” the Engineer in Charge may also require
additional precautions to be taken at the worksite. These may include safety wires, barriers and other
protective devices.

5.5 When the limit of approach to exposed live electrical apparatus including overhead lines is more than
6.4 metres allowing for the most adverse conditions of sag and sway then a Permit to Work

is not required. However, if there is any doubt that an advertent infringement may occur, then the
Engineer in Charge shall be contacted by the person in charge of mobile plant operations.

5.6 Control of suspended loads shall be achieved by means of dry fibre rope or other approved insulating
medium and neither the load, any conducting rope or the mobile plant, shall be touched by hand by
any person outside the cabin while the mobile plant or any conducting object attached is in motion.

5.7 During operation of the mobile plant, only those persons at ground level actually involved in the work
associated with the mobile plant shall be in the vicinity of the plant; all other employees shall remain
clear.

5.8 A sticker reading “Danger - Watch Out for Overhead Power Lines” shall be maintained in a prominent
position to the view of the driver/operator in the cabin of all mobile plant which has a jib or extensible
portion which could reach overhead conductors.

6.0 General

The limits of approach given in this appendix shall be the distance from the person, mobile plant (including
elevating work platform), vehicle or object or anything in contact with the person, mobile plant, vehicle or
object to the nearest conducting component of the exposed live electrical apparatus. Allowance shall be made
for the most adverse conditions of sag and sway.
