3.Is adequate lighting available throughout the accommodation ?

4. Is the accommodation heating and air conditioning system working


efficiently?

5.Are the officers and crew public sanitary facilities (WC, Lavatories, showers,
etc..) working in good order and clean ?

6. Are crew cabin bathrooms, water closets and sinks operating properly ?

7. Is water pressure sufficient to the bathrooms and water closets ?

8. Are cockroaches and other insects properly dealt with ?

9. Is hot and cold water available ?

10. Is there abnormal Vibration at any working or resting area which


unconfortable for human factor

l. Is there abnormal Noise at any working or resting area which unconfortable


for human factor.

11 . Is there abnormal Temprature at any working or resting area which

12. Is the areas free from smoke cigarettes except designated smoking rooms

13. All crew cabins are tidy and clean, no any open food kept in cabins,
garbage bin available, ligtinings and equipments working properly, furnitures,
clothes lockers, drawers are intact and bed including sheets, blankets etc...
are in good order.

14. Is the Hospital and all content in good working, clean and tidy condtion
including the bed, bath, lighting, ventilation fan, etc....

15. Hospital alarm are marked and working properly.

16. All Oxygene resuscitator sets are working properly , cylinders at the
sufficient level and instruction to use posted near the set.

17. First Aid kit available for emergency response and there is no any expired
medicine in the Pharmacy

18. Medical Logs kept properly and no any conflict with Accident Reports.

19. Is there hand grab / holding bar inside the shower cabins and in good condition

20. Is there non slip cloth at bottom of shower cabin and in good condition.

21. Stopper of doors in Accommodation and crew cabin are in good condition
and able to hold door open when at greater rolling angles, at heavy weather
condition.

22. Ensure main and emergency lightining in the cabins are working properly.

23. Ensure sanitary facility (WC) are working properly in navigation bridge
and ECR.

24. Ensure Laundry facility, laubdry machine, drying machine, ironing


boardand iron working properly.

25. Confirmation of all spaces and rooms are being used for what they are
intended for.

26. Crew recreation facilities including bookcase, and facilities for reading,
writing and for games are in good condition.

27. Are records available to confirm that shower heads (where fitted) are
cleaned and disinfected every 3 months?
28. Are records available to confirm that the fresh water loading hoses are
being regularly disinfected?
29. Are records available to confirm that the fresh water storage tanks have
been cleaned and disinfected within the last 12 months?

2. The engine room and other machinery spaces are generally examined YE NO N/A
by 2nd Eng. S

1. Are all engine rooms bilges clean and free of oil ?

2. Is the steering gear compartment clean and free of oil and any kinds of
garbage ?

3. Is the engine room machineries are clean and free from oil/fuel leaks
icluding accumulation of oily rags ?

4. Are the E/R walking plates/gratings fixed properly, clean and free fm any
oily mixtures.

5. Are acetylene and oxygen cylinders stored outside of the accommodation,


engine room and other machinery spaces, as seperated ?

6. Is paint and thinners stored outside of Accommodation and machinery


spaces in designated storage rooms ?

7. Is the machinery, pumps, shafts, fitted with protective devices (guards) as


considered necessary

8. Maneuvroing Area of the Lifting Appliances are tidy, clean and in good
order

9. Machineries, Bunker and Oil tanks drip trays are clean and empty and
plugged properly

10. Fuel and Bilge overflow/ventilation drip trays are clean, empty and pludged
properly?

11. Oil/fuel tanks level indicator glass self closing devices are working and not
obstructed

12. Fuel/oil tanks sounding pipes self closings are working and not obstructed

13. First Aid kit, Immersion suits, EEBD's and Life jackets are in good
condition and no any expired item.

14. Equipmenst in Engıne store room are well lashed in shelves as sea
secure.

15. Is adequate lighting available throughout the stairs ?

16. Stairs in ER are in good condition, steps structure are non slip type or non
slip painted, steps are in a contrast light colour with ground, first and last
steps are painted with yellow to incresae visibility .
3. The food and catering areas were generally examined by C/O : YE NO N/A
(Ref ILO 68 and 147) S
1. Confirmation that the catering equipment and facilities including food preparation areas and
galleys are in satisfactory condition and adequate for preparing hot meals in all sea and weather
conditions.
2. Plateas and oven cleand and free fm any oil

3. Oven and all available plate cells working properly

4. Galley cooking aspirater Vent suction side mesh and outlet fan head flame
arrester is clean and free fm any oil traces, Lighting is adequate at in Galley,
pantry and strores.

5. Areas between table and the bulkhead have cleaned in detail, there is no
any oil and food remaining between. Washbasin/ under washbasin areas and
preperation table are satisfactorly clean.

6. Are crew provisions suficient and in satisfactory condition with no


spillage, no rotten food or unsanitary conditions in stowage and there is no
accumulition of ice in the storages.

7. Refrigerated provisions store rooms found clean and of adequate size for
the provisions and refrigeration machinery considered capable of maintaining
the provisions at adequate temperatures ? ( meat/fish stores should be below
-17C)

8. Pantries are clean and tidy and public refrigirators, toast machines, kettle,
tea machines are clean and working in good order

9. Are water taps in working conditions and water colourles/odourless?

10. Knifes and and sharp tools secured in a lockable shelve

11. Verification that the shielding devices of naked flames are satisfactory (if applicable)

12. Using of LPG bootles for galley is not allowed. Verfiy no LPG bootles in use for Galley
service.

13. Verification of the galley’s cleanliness, special attention being given on areas around
furniture and appliances which are not flush with walls and floors

14. Confirmation that furniture and accessories are made of a corrosion resistant material.

15. Verification that the floor in galleys is in satisfactory condition and made of impervious
material

16. Confirmation that the fresh water hose connexion available for washing down is supplied.

17. Confirmation that drinking is available at galleys and near the crew mess room

18. Verification that the cold rooms / stores the doors can be opened from inside spaces and the
push button installed inside activates an alarm easily noticed by the crew.

19. Confirmation that the insulation, refrigeration arrangements and machinery in the cold rooms
are in satisfactory condition and capable of maintaining the provisions at an adequate
temperature at all time

20. Confirmation that shelves, bins and drawers in the stores are clean and in satisfactory
condition

21. Confirmation, dry provision, vegatable and fruits, kept fresh conditions
and all expired these shall be immediately removed and disposal .

22. Confirmation fish and meat refrigeration room floors (bottom/wall)


without ice surface

23. Confirmation all refrigerators (messroom/galley/cabins) with all gaskets


in satisfactory conditions without any damage

24. Vegetable and fruits shall be kept in refrigerated provisions store rooms.

Confirmation found clean and of adequate size for the provisions and
refrigeration machinery considered capable of maintaining the provisions at
adequate temperatures ? ( Vegetable/fruits should be kept 4-6C)
4. Deck And other external areas were generally examined by C/O : YE NO N/A
( Ref IL0134 and 147 & ISO 18001) S
1. Area of the Lifting Appliances, Windlass and mooring Winches are clean
and in good order ?

2. Is the accommodation ladder area clean, tidy and in good working order

3. Walking Gratings on Cat Walk, platforms, P/R and mooring areas are fixed
properly and not rusty?

4. Main deck is free from oily mixtures and no any slippery surface?

5. There is no any rust and any kinds of waste leak traces fm scupper plugs,
and other area on Hull ?

6. Area of the Lifting Appliances, Cranes, Windlass and mooring Winches are
clean and in good order

7. Mooring areas are non slippery surface and free fm any oil?

8. P/R and Bow thruster bilges are clean and free fm any oily mixtures?

9. No any oil leaks fm cargo, ballats, BW and other pumps, valves, lines and
connection

10. Windlass driptray are clean and able to pludging properly

11. Cargo Drip Trays are emty and clean and able to be pludged properly.

12. Pilot Emberkation area is marked proıperly and walk ways fm pilot point to
accommodation is non slip surface.

13. Ladder's from Catwalk to deck are in good condition, steps structure are
non slip type or non slip painted, steps are in a contrast light colour with
ground, first and last steps are painted with yellow to incresae visibility at
night.

14. There is no any mobile tools/equipment/stores on main deck. All mobile


stores, equipmensts on catwalk are properly fixed.

15. Drums and Cylinder are secured in fix foundations. Drums are covered
with canvas to protect fm sun and MSDS available. Structure of Drums are in
good condition.

16. Free Fall is in good cosmetical condition, bilge clean/dry and seats clean
tidy,

17. Rescue Boat is in good cosmetical condition, bilge clean/dry and seats
clean tidy,

18. Chain Lockers are dry and clear fm mud.

19. Forecastle is tidy and clean, all equiments are well secured, ladder is in
non slip condition, ligting &ventilation working very well.

20. Paint Locker clean and tidy, all paint bins are secured, shelves secure
bars in place, paint and Thinner bins covers are closed, ventilation working
properly, MSDS and PPE are in good condition.
