SCCP 850 Innovation Exercise:

Description:

1. This is a team assignment using the assigned groups found in SCCP 850 Blackboard
under Groups.

2. Individual members of a group should review the Ten Trends in Healthcare slides found
in Blackboard and select one trend to conceptualize an innovative concept.

3. The team should meet to review the various individual ideas and select one or two
concepts to be further developed into an innovative idea using the process described in
the October 6th lecture.

4. A summary of the group innovative idea should be developed containing less than 200
words. See example below for format.

5. One group member should document participation of all members in the summary
document. If there is a non-participant then please list the name(s) of the individual(s).

6. Submit the summary through Blackboard into your specific Group. Please indicate
FINAL in your submission file name if you use Blackboard to share draft documents
among the Group.

7. No credit will be given for any summaries received after October 12th deadline.

Concept (Idea) Title: Collaborative Practice with a Healthy Cafe

Description: We propose to implement a patient centered medical home that includes a

dispensing pharmacy, a doctor's office, an imaging center, a laboratory, and a healthy choice cafe
all in the same building. They would collaborate, meaning all charts could be accessed by each
department and the patients could see the doctor and get their medications filled all at one time.
Medical records could also be electronically accessed by patients via a portal and they could use
this to enhance communication with their healthcare providers. The doctor would actively
engage the pharmacist to get assistance on dosing of medications and education for patients with
chronic diseases. The café would serve healthy food and drinks and would have a healthy recipe
of the week free for patients. The main focus of this healthcare center would be to have the
doctor and the pharmacist collaborating together to treat the patient. It would also help with the
access issue that many residents of South Carolina face that limits them to showing up to
doctor’s visits and picking up their medications on time.
