PART 1 – GENERAL DESCRIPTION

INTRODUCTION

1. This User Requirement Document (URD) defines the requirements of the end user in order to ensure the delivery of fit for purpose
infrastructure at the Port Loco Ebola Treatment Unit (ETU), Sierra Leone.

BACKGROUND

2. The Ebola Virus Disease (EVD) continues to spread across West Africa with cases reported in Sierra Leone, Guinea and Liberia. The WHO
has requested international support in addition to the current work by NGOs who are running at full capacity. The UK has the lead for the response in
Sierra Leone, part of which includes the improvement of in-country support for containment and treatment. The ETU will be procured, delivered,
sited, equipped and constructed in country by DfID with Royal Engineer support for design and over sight of construction.

OUTPUT

3. The output is to provide a fit for purpose ETU, to me manned and operated by an NGO.

CONSTRAINTS

4. Construction is to be complete no later than 17 Nov 14.

5. Due to the urgency placed on the project, commercial decisions on procurement will be made by the Client.

6. All design and construction is to be safe, however, it is accepted that it is not possible to comply with UK construction regulations.

7. The following are constraints highlighted by the end user:

a. The site layout is to be dictated by clinical requirements, in particular the requirement to maintain the correct flow of patients, staff and
equipment to prevent contamination of non-infected personnel or areas.

b. The site is to be split into no-risk, low-risk and high-risk zones.

TIMING AND PRIORITY

8. The project is expected to the delivered by a civilian contractor including a locally employed civilian workforce. This will be supplemented with
Republic of Sierra Leone Armed Forces (RSLAF) Military Engineers, to conduct specific tasks.

9. DfID will directly purchase several key items, in particular tents, incinerators and generators. These items will be provided to the contractor for
erection and installation.

10. The contract will be put to tender on 7 Oct 14.

11. Construction is to be complete no later than 17 Nov 14.

JUSTIFICATION

12. This ETU is urgently required to help halt the spread of EBOLA, and thereby avert a global pandemic.

IMPACT STATEMENT

13. Delays to the construction of this facility will have a disproportionate impact on the spread of the disease.

ASSUMPTIONS

14. During the production of this URD, the following assumptions have been made:

a. Based on medical advice, the discharge from the septic tanks in the soakaways will contain an acceptable level of residual chlorine and
will not pose any threat to the environment.

b. The procurement and shipping mechanism will allow the required units to be delivered.

c. In order to pursue the most expedient commercial solution the contract will be put out to tender with incomplete technical information,
being the best that the MDA can provide by 6 Oct 14.

100 BED EBOLA TREATMENT UNIT REQUIREMENTS

Site Layout
1 General Site The entire site is to be surrounded by an outer perimeter fence with a gated single point of
Layout entry. This fence is to be 1.8m tall above ground and buried to a depth of 0.3m to prevent
animal access. The fence is to be chain-link or similar.
A perimeter track, inside the outer perimeter fence, is to provide vehicle access to the ETU.
2 ETU Layout The ETU is to be split into three zones:
 Red Zone (High Risk). For Ebola patients and staff in full PPE only.
 Green Zone (Low Risk). For staff conducting work and transit to and from the Red
Zone.
 White Zone (No Risk). Administrative area for staff, including storage, office space and
staff catering facilities.
Water
3 Water Quality The ETU requires:
 Raw (un-treated) water for patient showers.
 Low chlorine (0.05%) water for general use.
 High chlorine (0.5%) water for cleaning.
4 Water Quantity The ETU requires 400L of water per day per patient, for a total of 100 patients. This figure
includes allowance for staff usage and will be split between Raw, Low Chlorine and High
Chlorine.
The ETU is to have water storage for a minimum of 3 days of supply.
An additional pump of each type used is to be purchased and provided to the end-user as
spares.
Power
5 Generators Power to the ETU is to be provided by 250 kVA diesel generators and is to be independent of
any local supply.
A minimum of two generators are required in order to provide redundancy and allow
maintenance.
6 Power and Power and lighting is required throughout the ETU, as stipulated for individual locations.
Lighting Every ward is to have a double socket on each tent “leg” at a height of 1.2m.
The Dining Hall is to have a double socket on each tent “leg” at a height of 0.5m.
External lighting is to be provided at: all entry and exit points; visitor areas; generator farm;
burns pit and incinerators; reception area and ambulance washdown; morgue viewing area
and ambulance bay.
Waste disposal
7 Incinerator Two medical incinerators are to be installed in the Red Zone for the disposal of all clinical
(Red Zone) waste.
Incinerators must be operable by personnel in full PPE, front loading incinerators are
preferred to ensure safe loading.
Incinerators must be refuelled from outside the Red Zone.
Road to be extended up to incinerator pad, double fence is to cross the road and have a
lockable double gate.
8 Burns Pit There is to be a burns pit in the Red Zone, for burning of all non-medical waste produced
(Red Zone) within the Red Zone, and as a back up to the incinerators.
9 Ash Pit There is to be an ash pit within the Red Zone, in close proximity to the incinerators and burns
(Red Zone) pit, for the disposal of all ash produced within the Red Zone.
10 Burns Pit There is to be a burns pit in the White Zone, for disposal of general waste.

11 Drainage The red zone is to have a separate drainage system.
(Red Zone) All waste water from the red zone is to be treated as “black” water to drain into a single septic
tank.
The red zone septic tank is to have its own separate soak-away, a minimum of 150m down-
flow from any boreholes.
12 Drainage The white and green zones may have a shared drainage system, with a septic tank leading to
(White and a soak away.
Green Zones)
High-Risk Zone (Red Zone)
13 Layout The Red Zone is to be segregated into three areas (1, 2 and 3), patients, staff and equipment
(Red Zone) can only ever transit from a lower area to a higher area (i.e. maintaining flow from low
contamination to high contamination).
All tents and structures are to have 1m of aggregate laid around the outside as a
walkway/apron.
Aggregate is to be laid at the visitor areas, both inside and outside the fence.
14 Fencing There is to be double fencing all the way around the Red Zone and between Red Zone Area
(Red Zone) 1, Red Zone Area 2 and Red Zone Area 3.
There is to be a 2m clear gap between fences.
Fences are to be see-through and un-imposing in order to avoid the feeling of imprisonment.

15 Laboratory The laboratory is to be provided by others. The contractor is to provide:
 A 10x10m concrete slab with:
 Raw water supply.
 Single phase power supply.
The pad is to span between the green zone and Red Zone 1 (Suspected Cases Area). The
laboratory slab is to be adjacent to the edge of the site to enable placement by crane later.
16 Patient Entrance There is to be a weather-tight triage area providing the access from patient arrival area into
Red Zone 1 (Suspected Cases Area). It is to have:
 Lighting.
 A cleanable floor.
 An initial patient shower with raw water, for washing prior to entering the triage area.
 A supply of high chlorine water for disinfecting.
 Sufficient space for 8 x suspected patients to sit, each a minimum of 2m from any
other patient.
 Space for a doctor behind a desk.
17 Walkways There are to be cleanable walkways connecting all structures within the red zone.
Red Zone walkways are to be covered to protect staff wearing PPE from rain, providing a
minimum of 1.8m clear walkway.
18 PPE Dressing There is to be a weather tight area with lighting, providing a minimum of 20m 2 working area
Station for personnel to don PPE prior to entering the Red Zone. This structure is to be in the Green
Zone, with a clear gap leading to a double gate, which is the only route into the Red Zone.
19 PPE Removal Two separate weather tight areas with lighting each providing a minimum of 20m 2 working
Station area for personnel to remove PPE on exiting the Red Zone. It is to have:
 A cleanable floor.
 Drainage into the Red Zone drainage system.
 High chlorine water supply.
 Providing the means of access from the Red Zone into the Green Zone.
Red Zone Area 1 (referred to as Suspected)
20 Suspected Ward The “Suspected” patients ward is to:
 Provide a weather-tight shelter, with a floor area of at least 200m 2
 Have a cleanable floor, draining out of the shelter into the Red Zone drainage system.
 Have 2 x water points evenly, spaced within the shelter, providing Raw, Low-Chlorine
and High-Chlorine Water.
 Have a double power socket attached to each tent “leg” at a height of 1.2m above the
floor.
21 Ablutions The “Suspected” patients area is to have ablutions, which are to:
 Have 4 x Asian pan toilets.
 Have 4 x Raw Water showers.
 Have Lighting.
 Have a High Chlorine water supply for cleaning and flushing of toilets.
 Be easy to clean with no unnecessary details.
22 Visitor Area There is to be a designated visitors area, allowing patients to talk to visitors outside the area
through the 2m double perimeter fence. There is to be a small shelter for patients, but there
must be no means of physical contact between patients and visitors.
Red Zone Area 2 (referred to as Probable)
23 Probable Ward The “Probable” patients ward is to:
 Provide a weather-tight shelter, with a floor area of at least 300m 2
 Have a cleanable floor, draining out of the shelter into the Red Zone drainage system.
 Have 2 x water points, evenly spaced within the shelter, providing Raw, Low-Chlorine
and High-Chlorine Water.
 Have a double power socket attached to each tent “leg” at a height of 1.2m above the
floor.
24 Ablutions The “Probable” patients area is to have ablutions, which are to:
 Have 4 x Asian pan toilets.
 Have 4 x Raw Water showers.
 Have Lighting.
 Have a High Chlorine water supply for cleaning and flushing of toilets.
 Be easy to clean with no unnecessary details.
25 Visitor Area There is to be a designated visitor area, allowing patients to talk to visitors outside the area
through the 2m double perimeter fence. There is to be a small shelter for patients, but there
must be no means of physical contact between patients and visitors.
Red Zone Area 3 (referred to as Confirmed)
26 Confirmed Ward The “Confirmed” patients ward is to:
 Provide a weather-tight shelter, with a floor area of at least 300m 2
 Have a cleanable floor, draining out of the shelter into the Red Zone drainage system.
 Have 2 x water points, evenly spaced within the shelter, providing Raw, Low-Chlorine
and High-Chlorine Water.

30 Patient There is to be a patient discharge shower, leading from Red Zone Area 3 into the Green
Discharge Zone, this is for recovered patients to exit the facility. The patient discharge is to have:
 A raw water shower.
 Drain into the Red Zone drainage system.
 Provide cover from view for showering and dressing.
31 Mortuary There is to be a mortuary which is to:
 Have storage area for a minimum of 10 bodies.
 Have a designated viewing area, for visitors outside the perimeter of the Red Zone to
view bodies prepared for burial.
 Have lighting.
 Have a High-Chlorine water supply for cleaning.
 Be easy to clean with no unnecessary details.
 Drain into the Red Zone drainage system.
 Be lockable.
 Have Air Conditioning.
32 Ambulance Bay There is to be an ambulance bay with direct access into Red Zone Area 3 for the collection of
bodies and delivery of confirmed patients, it is to:
 Provide access through a double gate in the double fence, with inner gates opening
inwards and outer gates opening outwards.
 Have hard-standing, draining into the Red Zone drainage system.
 Have a High-Chlorine water supply for cleaning.
 Be covered, to protect from rain when loading bodies.
 Have lighting for use at night.
33 Visitor Area There is to be a designated visitor area, allowing patients to talk to visitors outside the area
through the 2m double perimeter fence. There is to be a small shelter for patients, but there
must be no means of physical contact between patients and visitors.
Green Zone (Low-Risk)
34 Locker Room Access from the white zone to the green zone is to be only through the “locker rooms”. These
are to be gender specific weather-tight structures with lighting, each is to contain:
30 x small “swimming pool” style lockers, each capable of taking a padlock.
Hanging area for safety clothing.
35 Pharmacy/ A secure and weather-tight structure with lighting, providing:
Dispensary Lockable door to/from the white zone.
An access window into the green zone, but no means of direct access for personnel.
Temperature control.
Cleanable floor and walls.
36 Laundry A weather-tight laundry area with lighting providing:
A 6.5 x 8m tent for washing machines and driers.
A 6.5 x 8m tend with the sides up, on a concrete slab with drainage.
A further 6.5 x 8m slab with drainage.
