AFFIDAVIT OF LEGAL BENEFICIARIES

I, MELVIN P. MENDOZA, of legal age, single, Filipino and resident of

Brgy. Lumipa, Gen. E. Aguinaldo, Cavite, Philippines, after having been duly
sworn to in accordance with the law, depose, and state that:

That I am an applicant for enlistment at the Armed Forces of the

Philippines;

That I am the son of Mr. Rubencio P. Mendoza & Estelita Mendoza of

Brgy. Lumipa, Gen. E. Aguinaldo, Cavite;

That I am naming my parents as my legal beneficiaries as required by my

application with the Armed Forces of the Philippines;

That I am attaching my birth certificate to attest the truth of my filiation

to my parents;

That I am executing this affidavit to attest to the truth and veracity of

the foregoing facts and for all legal intents and purpose that it may serve;

IN WITNESS WHEREOF, I have hereunto set my hand this 26th day of
