I, BERNADETTE POJAS JULIANO, of legal age, Filipino, and a resident of

Barangay San Agustin, Hagonoy Bulacan, after having been duly sworn to in
accordance with law, depose and state:

1. That I am a solo parent to my two (2) sons who are living with me and my
parents at our residential home in San Agustin Hagonoy Bulacan;

2. That I am left alone with the responsibility of parenthood since my partner

was a non-Filipino and whose whereabouts could not be located from the
country where I have worked as an Overseas Filipino Worker; and

3. That I am executing this Affidavit to attest to the foregoing facts and for all
legal intents and purposes.
