PROJECT TITLE : FABRICATION OF (1) UNIT SS DUMPING TABLE AND FOOT STAND
LOCATION : PACKING EXPORT - MAIN PLANT

SCOPE OF WORKS:

The contractor shall supply labor, materials, consumables, tools, equipment, technical supervision and other
related forms/activities necessary in the completion of the following:

1. Fabrication of one (1) unit dumping table and foot stand:

a. The contractor shall fabricate the dumping table and foot stand similar to the existing
dumping table and foot stand of LINE W at packing export. Thus, every detail of LINE W
dumping table and foot stand should be copied such length and width, elevation, leg
adjuster, plate thickness, plate bending, bracing and support sizes, etc.
b. All materials to be used will be stainless steel.

2. Requirements:
a. The contractor shall specify the number of days to complete the fabrication after the
received of P.O.
b. All welds must be GTAW. Apply tigklen to clean the welded areas.

3. Housekeeping, disposal of unused items, and general cleaning.

SPECIFICATIONS AND PROVISIONS:

1. The contractor is required to visit the site/project location during shutdown to measure the
specifications of LINE W dumping table and foot stand and to evaluate the entire work. Picture
taking is not allowed.
2. The contractor shall ask an approval to the project engineer if in case there will be a revision of the
project.
3. Fabrication and assembly works shall be done in the contractor’s shop. Welding and joinery works
shall be of standard quality and shall be performed by TESDA-certified welders.
4. The contractor shall observe proper safety practices at all times during the entire
construction/installation period. ALL WORKERS SHOULD WEAR PROPER PPE.
5. The contractor shall ensure the safety of company’s equipment during the duration of the project.
6. The contractor shall be responsible for the safety of its employees, tools and equipment during the
duration of the project.
7. The contractor shall be responsible for all wastes and garbage disposals in accordance to the plant’s
GMP rules and regulations. The area shall be left cleared and clean after the completion of the
project.
 8. The contractor shall be required to field out one project engineer/coordinator and one safety officer
to supervise the project at all times.
9. Deadline of submission of quotations shall be on JUNE 05, 2017, MONDAY, @ 5:00 PM. Late
submission shall be considered disqualified.
