I, ELIZABETH J. SARABIA, Filipino citizen, of legal age, widow and

with present residence and postal address at Purok 118 Brgy. Mendez
Crossing East, Tagaytay City after being duly sworn to in accordance with
law, hereby depose and state:

That I the daughter of the deceased ELENA SARABIA who is a registered

subscriber of Tagaytay Water District with Account Number 006-03763
located at Mendez Crossing, Tagaytay City;

That I am the occupant of the house even when the deceased was still alive
and so, I am the one who is regularly paying for the bills including the water
dues;

That the real name of my mother is ELENA SARABIA and all her records
reflect her correct name except the registered name in the Tagaytay Water
District where it has been FELINA SARABIA, which might be a clerical error
only during the time of her application to which she was not able to rectify it
during her lifetime;

That I am sure of this fact because I have been paying for the water dues
for more than ten (10) years already under the name of FELINA SARABIA;

That both names pertain to one and the same person as referring to ELENA
SARABIA;

That I am executing this affidavit in good faith and without any intention to
defraud or deceive;and

As such, I am executing this Affidavit to attest to the truth of the foregoing

specifically for Tagaytay City Water District purposes and for other
reliefs as may be just and equitable.
