This Product Comparison covers bed, chair, and wheelchair alarms that detect when a patient has fallen out of or attempted to exit the bed, chair, or wheelchair. Also included are
pressure-sensitive mats placed on the floor, bedside infrared motion detectors, alarms built into self-releasing belts (e.g., bed, chair, and wheelchair belts), cords and garment
clips, and patient-worn alarms (e.g., leg cuffs). Alarms built into the bed are also included in this report. Alarms attached to walkers are excluded, as are alarms that alert staff
when a patient has already fallen (i.e., fall-detection alarms) and systems that alert staff when a patient has left the authorized area.
Patient falls usually occur in either the patient's room or the bathroom; they most often happen when the patient gets out of his or her bed or chair unassisted. Facilities have a
duty to exercise reasonable care to prevent injuries and to ensure a secure environment for all patients. Occupancy alarms can help reduce the likelihood of falls and can promote
speedy assistance to patients who have already fallen by alerting caregivers that a patient who should not get out of bed unassisted is doing so.
Occupancy alarms are typically used on patients with cognitive dysfunction (e.g., dementia) or dizziness, or on patients whose medication increases their risk of falling. Monitoring
patient occupancy allows more freedom for patients and healthcare personnel and can make patients feel more independent. Patient-occupancy devices are primarily used in
long-term care facilities, such as nursing homes and assisted-living facilities, as well as in acute care areas and long-term care areas in hospitals.

Purpose
Patient falls usually occur in either the patient's room or the bathroom; they most often happen when the patient gets out of his or her bed or chair unassisted. Facilities have a duty
to exercise reasonable care to prevent injuries and to ensure a secure environment for all patients. Occupancy alarms can help reduce the likelihood of falls and can promote speedy
assistance to patients who have already fallen by alerting caregivers that a patient who should not get out of bed unassisted is doing so.
Occupancy alarms are typically used on patients with cognitive dysfunction (e.g., dementia) or dizziness, or on patients whose medication increases their risk of falling. Monitoring
patient occupancy allows more freedom for patients and healthcare personnel and can make patients feel more independent. Patient-occupancy devices are primarily used in long-
term care facilities, such as nursing homes and assisted-living facilities, as well as in acute care areas and long-term care areas in hospitals.
Principles of Operation
Alarms include a monitoring sensor and often include a separate control unit and a power supply. Bed-exit alarms may be stand-alone units, or they may consist of preinstalled
sensors that are built into the mattress or the bed frame. Sensors are available in several configurations: mats placed on or under a bed or chair, mats placed on the floor next to a
bed, chair, or wheelchair; infrared motion detectors; or a security belt, garment clip, or leg cuff.
Occupancy alarms allow patients to maintain a free-movement zone, or an area adequate for normal bed or chair activity. If the patient leaves the free-movement zone, an alarm will
sound in the room and/or at the nursing station, in a hallway, or on a staff pager via a nurse call system interface or a remote receiver console.
The control unit is secured (by Velcro, hanging pouch, or mounting bracket) directly to the bed, chair, wheelchair, IV pole, or wall. Some control units may be moved (e.g., from a bed to
a wheelchair) as the patient is transferred. Some may plug into an existing nurse call system or transmit a wireless (e.g., radio-frequency [RF]) signal directly to a central system.
Control units can be battery and/or AC powered. Battery-powered alarms usually have an audible low-battery warning that indicates when the batteries need to be changed. Battery
life varies significantly from device to device and depends on monitoring time and how often and for how long the alarm is triggered.
For in-bed or in-chair systems, a pressure-sensitive mat is laid down before the patient is placed in the bed, chair, or wheelchair. Floor mats are placed next to a bed, chair, or
wheelchair (where a patient would stand while attempting to exit). Mats are usually made of polyurethane, polyethylene, or Naugahyde and are available in various sizes and
thicknesses. They may be reusable or disposable.
Pressure-sensitive mats are placed below the pelvis or shoulders, or both, in the center of the bed and may cover a portion of the bed or the full width of the bed. Depending on the
supplier, the pressure-sensitive mat is placed either above the mattress (below the bedsheets and the mattress pad) or below the mattress; units placed above the mattress are
often disposable. Using several pressure-sensitive mats to monitor extremely restless patients may help minimize false alarms. On devices that allow the simultaneous use of two or
more mats, the alarm will trigger only when all sensors detect an absence of weight. In a chair or wheelchair, the pressure-sensitive mat is placed in the center of the seat; a seat
cushion may or may not be placed on top of the mat.
Once the pressure-sensitive mat is in place, it is connected to the control unit. If the patient's weight is lifted off the pressure-sensitive mat in the bed or chair or placed on a floor
mat, an alarm will sound. Some models have an alarm delay (e.g., three to five seconds) to minimize nuisance alarms.
Belt systems have release sensors built into the security belt of a bed, chair, or wheelchair. When the patient releases the belt, an alarm will sound.
Garment clips use a fixed or adjustable-length cord that attaches to the patient's clothing on one end and plugs into a control unit mounted on the bed, chair, or wall at the other end.
If the patient leaves the bed or chair, the cord is pulled out of the control unit, initiating an alarm. Depending on the supplier, the amount of force and the angle of release required to
initiate an alarm will vary. Some units' cords are color-coded depending on their length; customized cord lengths are available. Typically, shorter cords are used for chairs and
wheelchairs and longer cords are required for beds.
Bedside infrared (IR) motion detectors can be installed along one or both sides of the bed. The alarm unit is activated when the IR beam is broken by the patient attempting to exit
the bed or rise from the mattress.
Patient-worn alarms are attached to the patient by various means and are triggered by changes in position or motion. Leg cuffs, for example, are placed around the patient's upper
leg and will alarm if the patient attempts to stand.
Some manufacturers offer systems that can be configured for various sensor types (e.g., pressure-sensitive mat, IR beam motion detector, cords, garment clips). Facilities should
select occupancy alarms that meet the needs of their patients. In general, bed, chair, and floor mats are good choices for uncooperative or confused patients (e.g., dementia
patients) because the mats can be discreetly placed. Belts, clothing clips, and leg cuffs are better choices for restless or lightweight patients because nuisance alarms are less likely.
Infrared motion detectors are a good choice for most patient types but facilities may want to supplement motion detectors with another type of sensor for patients who are high fall
risks.
Alarms
Alarms will vary depending on the supplier. Some models may offer audible (e.g., tones) and visual (e.g., flashing LED) alarms generated directly from the control unit. Alarm delays can
usually be configured by the care giver and may range from 1 to 12 seconds. Alarm activation should be tailored to individual patient behavior (e.g., less sensitive sensors and longer
delays for restless patients). Depending on the unit, the reset feature can be either manual (e.g., latching) or automatic (e.g., non-latching). A non-latching alarm will reset itself if the
patient returns to the necessary position (i.e., sits back down in chair); a latching alarm will sound until it is reset by the caregiver. Healthcare personnel can press a hold button that
allows them to move patients without activating an alarm; on some units the alarm will automatically reset after the patient is returned to the bed or chair. Audible alarms may change
tones depending on the time of day so that other patients are not disturbed. Visual-only alarms (mute mode) are also available; these should only be used if there is a remote alarm in
place.
Control units vary in their programming capabilities. Some units cannot be programmed at all, as is commonly the case with leg-cuff and cord-activated alarms. Some units may allow
family members or friends to record a personal alarm message, which may help relax patients by allowing them to hear the voice of someone they know. For example, an elderly
patient coming out of surgery attempting to exit the bed or stretcher might have a message from his daughter saying, "Dad, do not get up; you just had surgery. The nurse will be right
in to help you." When using voice recordings, users should consider the patient's mental stability.
Some models may allow the user to track alarm information such as patient name or ID, alarm history, notes, cumulative time of service per patient, and response times. Some units
work in conjunction with personal computers for report generation.
Reported Problems
Some bed-exit alarms do not immediately reset after sounding. During the reset process, which may take several seconds, there may be no indication that the alarm is not activated.
If a patient exits the bed during this reset interval, the alarm will not sound, increasing the likelihood of patient injury.
For line-powered units, users should verify that cords do not create trip hazards. If electrical cords are exposed, users may accidentally disconnect the occupancy alarm.
If sensors are placed under a mattress that is too heavy, the sensor may not detect the patient leaving the bed. Lightweight patients may not be detected at all by the sensor, or it
may alarm every time the patient moves around in bed.
For battery-powered units, battery contact may be lost if the unit is dropped from a bed or chair.
Units with clothing clips should be used with caution because it is possible for patients to remove the clip from their clothing or remove clothing and exit without initiating an alarm.
Some units may have problems releasing if the cord is pulled from an angle. Also, users should ensure that cords cannot become entangled around the patient's limb or neck, or
entangled around furniture which could create a tripping hazard.
Users should determine the monitoring alarm parameters before use to reduce the number of false alarms. If the time delay is not properly set, alarms could be triggered
prematurely. The alarm delay should be reconfigured for each patient depending on the patient's behavior (e.g., restless, prone to wander).
Users should inspect pressure-sensitive mats during use and reusable mats between uses to verify mat integrity. Mats may be loosened, bent, and twisted by restless patients. If a
mat becomes loose and is bent, the internal wiring can be separated or frayed, possibly risking monitor malfunctioning.
Users should follow the manufacturer's recommendations for cleaning mats. Some mats are heat-sealed or have an optional heavy-duty cover that can withstand repeated cleanings.
Cleaning mats with soap and water should not affect their integrity; however, disinfecting and/or sterilizing them may cause them to degrade faster. User facilities should inspect pads
periodically. Products, particularly disposables, should not be used beyond their useful lifetime.
Some patients may be sensitive to latex; if a mat contains latex, users should ensure that those patients do not come into direct contact with latex components.
In the United States, the Occupational Safety and Health Administration recommends that audible alarm volumes not exceed 90 dB. Since occupancy alarms have the capacity for
frequent and/or prolonged alarms, users should review the audible tone levels of the available alarms. If personnel are likely to be out of the audible range, users should consider an
interface with a nurse call, remote annunciation, or paging system.
It is important for facilities to keep in mind that occupancy alarms are meant to serve as an early-warning system only. These systems help staff reach patients at risk for falling
sooner than they would otherwise. These alarms cannot be used as a substitute for staff supervision and other fall-prevention programs because they cannot prevent an at-risk
patient from falling. The bed exit alarm system must be used in conjunction with a sound risk assessment and protocol.
Purchase Considerations
ECRI Institute Recommendations
Included in the accompanying comparison chart are ECRI Institute's recommendations for minimum performance requirements for occupancy alarms.
Alarms with both audible and visual functionalities are preferred. Alarms differ from one to another mainly in sensor configuration; each facility should assess which sensor
configuration is the best for its patients. Sensor preference will depend on how mobile patients are, their mental status (e.g., confused, heavily medicated), and their level of
cooperation. Clips, bed and chair/wheelchair mats, and cuffs should last at least 30 days; reusable belts and floor mats should last at least one year. Floor mats should have beveled
edges to reduce tripping hazards. Any surface that touches a patient (e.g., chair mat) should be latex free and washable to reduce allergic reactions and cross-contamination.
Alarm features are also an important factor to consider when purchasing occupancy alarms. The alarm delay available on some bed-exit alarms can enable staff to more effectively
use alarms on highly restless patients by reducing the number of nuisance alarms. The alarm delay feature is required for bed and chair mats. Units that offer a hold mode are
recommended because caregivers will not have to remember to turn the occupancy alarm back on after attending to the patient.
It is also important to consider differences in remote alarm capability. For example, while a small, 15-bed rehabilitation unit would likely only need audible alarms that sound in the
patient rooms, a 60-bed long-term care unit may require additional alarms that can be connected to the nurse call system or linked to consoles in the hallways to ensure that alarms
are heard. Also, when using a remote alarm, facilities may find it useful if there is a mute mode feature. This would allow caregivers to disable audible alarms at the bedside to avoid
disturbing other patients in the room.
Batteries are most appropriate for portable units (e.g., those used in wheelchairs). Batteries may be disposable or rechargeable. If the battery is rechargeable, it should recharge in
less than 8 hours. The battery should operate for at least 1 month. The type of battery, operating time, and recharging time should be considered and compared to the hospital's
patient occupancy tracking requirements. Units should have an audible low-battery alarm. If frequent occupancy tracking is required (e.g., in geriatric facilities), line-powered models
are beneficial to reduce the costs of replacing or recharging batteries.
Other Considerations
Many units can be interchanged between beds, chairs, and wheelchairs. Multipurpose units allow healthcare facilities to standardize their patient occupancy equipment. Some patient
occupancy alarm systems can interface with current nurse call and paging systems.
The cost of an occupancy alarm will increase if it is interfaced with an existing nurse call system, remote alarm console, or paging system. Software for report generation can also add
to the cost.
Pressure-sensitive sensors can be either reusable or disposable. Reusable pads may be more cost effective for facilities that require frequent patient-occupancy monitoring. These
pads are sealed to resist damage from liquids (e.g., urine) and typically have an expected lifetime of 6 months to 1 year. Disposables are single-use pads that are sealed to resist
damage from fluids, with a typical expected lifetime of 30, 60, or 90 days. Some suppliers may offer slipcovers for their pads that can be removed and discarded or washed between
uses. Some suppliers use antimicrobial material in their devices to protect against cross-contamination; the lifetime of these pads varies among suppliers. Some facilities may reuse
single-use pads or use pads well beyond their intended lifetime to reduce costs. These facilities must address issues surrounding cleaning and disinfecting/sterilizing procedures,
pad/sensor reliability, and risk of patient injury should the sensors malfunction due to improper overuse.
Depending on the supplier, 24-hour technical phone support, field technicians, or return mail may be available to support the device in case of mechanical failure. Users should verify
suppliers' policies on repairing devices that are subjected to high levels of abuse. Some suppliers may suggest keeping an extra control unit in inventory in case one requires servicing;
also, loaners may be available.
The healthcare facility should develop risk management procedures for determining alarm parameters, identifying at-risk patients, and dealing with fall victims.
Stage of Development
Historically, patients at risk of falling while in a healthcare facility have been treated largely with the use of physical and chemical restraints (e.g., vest restraints, antipsychotic
medications). In the United States, laws and regulations limit the use of physical and chemical restraints, leading to greater use of technological advancements in patient monitoring.
Improvements in alarm functionality, durability, and efficacy are continually being made. Some alarms may have a different tone to alert nurse's when a high-fall-risk patient is moving.
There are also alarms that stop sounding when a patient stops trying to get up. Some models can interface with other safety and monitoring equipment throughout the hospital, and
devices are becoming sturdier and more user-friendly. Newer systems offer a silent mode—the system alarms at the nurse's station, but does not audibly alarm in the patient room—
that can enhance comfort for patients in shared rooms by reducing nuisance alarms. Researchers continue to explore the use of supplemental falls-prevention technologies, including
pressure-sensitive bed rails and polysomnography-based body movement monitoring.
