1. GENERAL

This document shall covers project execution planning for piping and mechanical work.

1.1 Introduction
The Project Execution Plan (PEP) is the operational document for the project. It is owned,
maintained and utilised by the Project Manager and Project Team to support the delivery of the
agreed project outputs.

1.2 Project Objective


Construct Piping Work for Pomala High Pressure Acid Leaching (PPAL) Project in the
designated project schedule, ensuring the safety, health, environment and quality are fully
inserted and properly implemented into construction projects.

2. CONSTRUCTION PLAN

During construction work PT. Baharu Penta Kencana (BPK) shall comply to Client Chiyoda-SAE /
Owner and local regulation.
Construction plan for executing the Piping and Mechanical Work, shall be refer to standard drawing,
typical installation drawing and below procedures as issued by Client / Owner : Detail procedure for
specific work shall be provides and approve by Client prior commencing each construction
activity.

3. ORGANIZATION CHART AND RESPONSIBILITY

BPK will arrange a Site Organization. Effective workable system will be strictly adhered by the Project
Team.
BPK Site Organization shall have a good coordination with Client / Owner. The Project will be
managed and supervised by Project Manager from Head Office - Jakarta and Construction Team
Work Force at site will be managed and supervised directly by Site Manager.
Site manager also will handle and responsible for all site coordination and communication both for
Technical and or Non-Technical issue.
Below are short descriptions of personnel responsibilities and function for each key personnel as
describe in organization chart;

3.1 Project Manager (PM)


Project Manager are responsible for planning, management, coordinating and financial control
of a project.
Project Manager ensure that a project is completed on time and within budget that the project’s
objectives are met and that everyone else is doing their job properly.

Project Manager oversee the project to ensure the desired result is achieved, the most efficient
resources are used. Other responsibility:
1. Organizing all manpower working on a project
2. Making sure the quality standard are met
3. Making sure all the aims of the project are met
4. Safety for all worker at site maintaining
5. Oversee all scheduling and planning

3.2 Site Manager (SM)


Site Manager are responsibility for supervising construction sites and running construction
projects.
Site Manager to prepare sites prior to commencements of construction work (to set out the site
and organize facilities), to plan projects and ensure that they meet agreed specification, budget
and timescale and to oversee construction work. Other responsibility:
1. Coordinate with Client and Owner and reporting progress
2. Supervising project staff
3. Maintaining Health, Safety and Environment condition
4. Maintaining Quality Control
5. Maintaining good relationship with all worker and local communicate

3.3 Construction Supervisor


Supervisor will oversee the construction activities that take place on the worksite and will be
involved in supervise of workers.
Supervisor will report to construction Superintendent all foreman and workers will take their
direction from the supervisor, who is responsible for knowing what work must be done and
applying resources to accomplish the work. Other responsibilities:
1. Supervisor will implement the construction schedule to all foreman and workers
2. Making sure the quality standard are met
3. Overseeing the accounting, costing and billing
4. Making sure all the aims of the project are met
5. Safety for all worker at site maintaining
6. Oversee all scheduling planning

3.4 QA/QC Inspector


1. He shall be responsible for the execution techniques and control with respect to this Method
Statement and reference documents.
2. Witnessing all Checks / Tests.
3. Maintaining records of calibration, inspection and testing equipment.
4. Documentation of test records and stage inspection.
5. Reporting and monitoring of non-conformance status.

3.5 Rigger Supervisor


1. Shall coordinate all lifting activities and ensure that only certified equipments and trained crew
are participated in the lifting.
2. Shall ensure that the ground conditions are safe for any lifting operation, and take measures
to rectify the unsatisfactory and unsafe conditions.

3.6 HSE Team


1. To make sure all workers working at site with safety manner. Below is the a few
mandatory program shall be carried out by safety department to make sure worker working
with safety manner but not limited;
• To give safety induction to all workers, no exception, before they are working at site for
the first time.
• To do tool box meeting in every working day in the morning time before worker go to site.
• To do general weekly safety meeting
• To do safety award program
• To issue an official warning letter to the worker who break the safety rule for two time after
they have advice for the first time.
• To do weekly safety patrol to the site with attendance by SM.
• To do weekly general cleaning in all working area including office, warehouse and
workshop area.
• To provide poster, sign board and etc. which contained safety advice and message to
the worker who read it.

3. To prepare safety daily and weekly report.
4. To prepare accident report (if any accident occur)
5. To make good communication and coordination to the client safety supervisor.

5. MOBILIZATION PLAN

5.1 Personel Mobilization


BPK will assign and mobilize manpower whose have good experiences working in Oil and Gas
and Petrochemical Plant Project from various region of indonesia such as; Jakarta, Gresik,
Balikpapan, Bontang, Indramayu and Sumatera.
Inspite of that BPK will also hire local manpower from surrounding project area at Jakarta -
Indonesia who meet the qualification for the certain of work.
BPK shall be responsible to perform pre-employment medical checkup for all personnel before
mobilization to project site.
The manpower will be mobilized partially considering project condition, material availability and
work progress.

7. HEALTH, SAFETY AND ENVIRONMENT

7.1 HSE TARGET


Zero accident and incident
No harm to our employee
No damage to environment

7.2 HSE PLAN


HSE plan will focus on participative programs that are geared towards the prevention of
workplace accident and occupational disease or environmental pollution. This program will
actively involve all workers to run operational activity according to HSE principles.
Prior the star the work BPK will develop HSE Plan, Jobs Safety Analysis and Risk assessments
in compliant with Client HSE regulation.

7.3 HSE Monitoring & Implementation


HSE Supervisor and Safetyman is to ensure to monitor HSE performance with relevant action.
Monitoring daily Man-hours report and incident reporting.
HSE Implementation through program:
1. Participate regularly on Tool Box Meeting in daily basis, weekly saf ety meeting and monthly
safety meeting with client.
2. Implementation housekeeping program.
3. Monitoring discipline on wearing Personal Protective Equipment.
4. Conduct HSE internal inspection.
5. Implementation safety communication trough safety banner, poster, notice boards, etc.
6. Management communication such as Management Visit or Walkthrough.

8. QUALITY PLAN

Quality Plan acts as guidance quality for the project and all the Project Management, Site
Management and Site worker of the project should adhere to the project quality plan. Quality
Plan describes the activities, standards, tools and processes necessary to achieve quality in the
delivery of a project. The Quality Plan mention the detail work as following:
1. Responsibility of Management

3. Quality Audit
4. Testing Work
5. Defect Management

8.1 Coordination and Communication


• The QA/QC Supervisor shall prepare overall Inspection and Test schedule for each job of
the project.

This schedule shall be based on and synchronized with field construction schedule. Close
communication and coordination between construction groups and quality group is required
for this schedule preparation, revision and progress updating.
The Site Manager and Project Control shall review this inspection schedule for concurrence.
The inspection schedule once approved by the Site Manager shall also be distributed to
Client quality control representative for reference.
• The QA/QC Supervisor shall consult with his counterpart Client quality control
representative pertinent to any complain or nonconformance fin dings, and propose for the
acceptable resolution.
The QA/QC Supervisor shall maintain close communication with their counterpart of Client
inspectors pertinent to daily inspection activities and the planned next one or two day’s
inspection.
The daily inspection activities for the next coming week shall be prepared on the Weekly
Inspection Schedule (based on the overall QA/QC Schedule) and submitted to the Client
relevant inspector for information.
Application for inspection witness by Client shall be forwarded at least one day before the
test / inspection date as reference.
Inspectors for their file at the latest one-day after the test/inspection completion date. All
original records shall be logged properly for later transmission by the Site Manager to Client
upon or after completion of the project; one complete set copy shall be handled over
to Client Quality Assurance Department.

9. PROJECT CONTROLING AND REPORTING

9.1 Forecast Controlling


Accurate schedule forecasting will be achieved through carefully monitoring of actual progress
achieved which will be compared with planned progress. These control procedures
will be utilized for Electrical work. All forecasting and controlling system will be computerized to
the fullest extent.
BPK will prepare the “catch-up” plan, if in case schedule delays prior to affect Overall
Construction Schedule. The plan will be discussed and agreed with Contractor.
BPK will close communication with Contractor during all phase of the construction work by means
of regular and timely meetings, submission of reports, documents and other pertinent
information.
Formal weekly and monthly meetings and reports shall be prepared and shall include all aspects
of the construction execution.

9.2 Procedure for Schedule Controlling


90 days Construction Schedule will be originated by Project Control Supervisor. The Site Control
Supervisor will coordinate work activities of different disciplines and check that the schedules
are in accordance with the Project Master Schedule.
Weekly Schedules will be prepared by each discipline leader in compliance with the 90 Days
Construction Schedule. It will be used for a daily control of the work items. Weekly Schedule
Meeting will be held between discipline supervisor and Client.

4.3 Procedure Manpower Controlling


BPK will furnish all workers for the execution of the Work at its sole risk and account and assume
full responsibility. Therefore, BPK shall pay rates of wage sand allowances, and observe hours
and condition of employment that are not less favorable than UMR (Upah Minimum Regional)
UMR rate is issued by regional Governor.
BPK will take all precautions necessary to protect own, Client and Owner personnel and worker
from any outbreak of illness of an epidemic nature during the execution of the Work.
BPK will observe and comply with all fire, safety, security, and other rules and regulations
prescribed by Client and shall be responsible for the observance thereof by all workers.
BPK will follow all Safety Procedures outlined under the HSE Plan. In all cases BPK is
responsible for knowing and understanding the Emergency Control Plan and any other Client
regulation now or hereafter in effect for safety of personnel and plant.
BPK will obtain and maintain the necessary site working permits in accordance with Client
current and future security policies and regulations.
BPK will be fully responsible for the proper behavior of all its workers, at the job-site, and it shall
take all necessary precautions to prevent any strikes or concerted action of any sort by such
personnel.

Initial overall manpower mobilization plan will be prepared based on initial schedule
analysis during the construction planning stage. Detail mobilization plans will be developed by
each discipline during the detailed construction planning phase.
Daily manpower utilization will be monitored using daily time sheets.
The manpower utilization will be summarized monthly and compared with the planned
mobilization and schedule requirements.
Based on actual progress achievements and future progress requirements, the BPK’s
supervisor staff will forecast the future manpower requirements by each craft to meet the
schedule targets.
Performance of each construction discipline will be strictly controlled by supervisors to
ensure efficient utilization of workforce at site.

9.4 Progress Controlling


Each discipline will prepare progress report based in daily and weekly work accomplishments
and will compare it with planned versus actual progress. These progress control documents will
be collected by Project Control Supervisor for use in monthly reports to Contractor.
Report actual progress of all discipline will be input to the site to site computer system. And the
total monthly progress will be calculated.
The overall monthly progress status and progress by each discipline will be plotted on
Construction Composite Progress Status and reported to Contractor.
Actual Monthly Progress will be compared against planned progress and corrective action will
be taken whenever it is necessary to improve progress.

5. SECURITY PLAN

Security staffs are authorized to conduct security inspections at all times, including search of
personnel, toolboxes, vehicles, etc, as considered necessary.
No person may while on site commit any of the following acts.
Introduce onto the site, use or process any alcohol or habit forming drugs :
• Be under the influence of alcohol or habit-forming drugs.
• Contravene prescribe with violence any other person.
• Assault or threaten with violence any other person.
• Introduce a firearm, camera or any dangerous weapon onto the site.
• Personnel business or advertising for personal gain, within the construction site is
forbidden.
• Theft or attempted theft or unauthorized possession of Subcontractor / Client Property.
General Rules:
• Identification badges shall be worn and displayed at all times.
• A vehicle permit may be applied for and issued at the discretion of the security.
• A gate pass is required for the removal of all tools, materials, and equipment from
the premises.

• Smoking is permitted at the designated areas.

6. DEMOBILIZATION

Demobilization of Manpower and Equipment shall be started automatically when construction acitvity
coming slowdown up to all Electrical and Instrumentation completely finished. However before it to
be carried out PT. BPK will inform to Client in advance for information and review.
