1. 研究生論文指導教授申請表、變更表或共同指導教授申請表，請簽章完成後送回系辦留存。

The application form for designating an advisor of a graduate thesis or for advisor replacement

should be signed and delivered to the departmental office for archiving.

2. 研究生若需更換指導教授，請另填論文指導教授變更表。並徵得原指導教授及新指導教

授同意簽章後，送回系辦留存。If a student needs to change his or her advisor, he or she should

complete the Application for Changing Thesis Advisor form. After it is approved with the signature

or seal from the original and new advisors, the application should be delivered to the departmental

office for archiving.

3. 研究生若需增加共同指導教授，請另填論文共同指導教授申請表。並徵得指導教授及共

同指導教授同意簽章後，送回系辦留存。If a student needs to increase his or her advisor, he or

she should complete the Application for Increasing Common Thesis Advisor form. After it is

approved with the signature or seal from the original and common advisors, the application should

be delivered to the departmental office for archiving.
