Notes:
1. Serum Ceruloplasmin levels may be normal in upto 10% of affected patient & reduced in 20%
of carriers
2. Test conducted in serum & Urine

Comment
Wilson disease is an autosomal recessive disorder caused by mutations in ATP7B gene; a membrane
bound copper transporting ATPase. Defective copper incorporation into apoceruloplasmin leads to excess

catabolism & low blood levels of Ceruloplasmin. Serum copper levels are usually lower than normal
because of low Ceruloplasmin. Urinary copper is elevated invariably in symptomatic patients & is an
important diagnostic tool. Patients may present with neurological manifestation, hepatitis (elevated
transaminases with or without Jaundice) or hepatic decompensation (elevated, bilirubin, reduced serum
albumin & coagulation factors, ascites, peripheral edema & hepatic encephalopathy)
Clinical manifestations are caused by copper toxicity & primarily involve the liver & the brain. Because
effective treatment is available, it is important to make the diagnosis early.
