Greetings from OHNAP!

The Occupational Health Nurses Association of the Philippines (OHNAP), Inc. will be holding its 270th Postgraduate Course
on Basic Occupational Safety & Health (BOSH) for Nurses from February 4 to 9, 2019 at exactly 8:00AM-5:00PM and please
come on time at the Richville Hotel, Edsa Mandaluyong City.

The six-day course is designed for all nurses as required by the Bureau of Working Conditions (BWC) of the Department Of
Labor and Employment (DOLE) and in compliance with Rule 1960 of the Occupational Safety & Health Standards requiring all
nurses in OSH to take up the Basic Occupational Safety & Health (BOSH) course.

In this regard, may we invite you to participate in this program to update your knowledge and skills in delivering quality and safe
nursing care in the service of the Filipino workers in the field of occupational health nursing and as initial requirements for
employment as an occupational health nurse.

The pre-registration fee is Php8,500.00 (NET OF TAX) for each participant up to November 30, 2018 &Php9,000.00(NET OF
TAX) after November 30, 2018. this includes meals for the duration of the course and all materials for the course. This fee is
regulated by the ASHTOP (uniform fees charge to all members).

Payments may be deposited in any Bank of the Philippine Islands (SA# 1433-0012-45) branch nearest to your place. All check
payments (company checks only) must be payable to the OCCUPATIONAL HEALTH NURSES ASSOCIATION OF THE
PHILIPPINES, INC. or OHNAP, INC. ORIGINAL DEPOSIT SLIP should be presented and surrendered during the
registration on the first day of the session in exchange for an official receipt to be issued by the registration committee. As soon
as payment has been made, please accomplish the registration form attached and fax it to (02)894-3049 together with the
duplicate copy of the deposit slip. REGISTRATION FORM FAXED AFTER November 30, 2018 will be considered on-site
registration and will be charged Php9,000.00 (NET OF TAX). WE ARE ON A FIRST COME FIRST SERVED BASIS,
RESERVATION IN FULL PAYMENT.
