I,MEDELYN CHAVEZ VENZON, legal age, Filipino, married and with

present residence and postal address at 30 Luksuhin Ilaya, Alfonso, Cavite
after being duly sworn to in accordance with law, hereby depose and state:

That I recently purchased a parcel of land from DAMIAN J. ALCAZAR

consisting of NINETY ONE (91) square meters out of the THREE HUNDRED
TWENTY THREE ( 323) square meters of LOT 4012 located at Luksuhin Ilaya,
Alfonso, Cavite which the latter has bought from its registered owner
MATIAS ALCAZAR;

That said Deed of Absolute Sale was docketed as Doc. No. 334, Page No. 79,
Book VIII, Series of 2018 by Atty. Eric De Castro Estrella last July 30, 2018;

That recently, I am in the process of application for the titling of the above
mentioned property;

That upon inquiry with the Provincial Environment and Natural Resources
Office at Cavite, I found out that there was an application of prior titling of
the registered owner of the property in 1985 which did not push through;

That upon further verification with the Register of Deeds of Tagaytay City, I
verified that there was already an Original Certificate of Title in their
records;

That I am executing this affidavit to request for the cancellation and

withdrawal of the above mentioned title in order to secure and process a
new one;

As such, I am executing this Affidavit to attest to the truth of the foregoing

and for other reliefs as may be just and equitable.
