This is to certify that TERESITA I. MACASAET of Bulaklakan, Lipa City is a

member in good standing of the Lipa City Tourism, Arts and Culture Council,
having been actively involved in all affairs for the upkeep, development and
promotion of Lipa’s Museo ng Katipunan for more than five years.
