 Eye/ Sight Assessment

1. Eye Assessment (Recognition of X and O with the Left Eye)

Distance of Card (X and O) Observation/s

4 inches away from the subject X and O are still recognizable.
3 inches away from the subject X and O are not
recognizable.
Diagnosis: The patient’s Left eye recognition of X and O is Normal.

Eye Assessment (Recognition of X and O with the Right Eye)

Distance of Card (X and O) Observation/s

3 inches away from the subject X and O are still recognizable.
2 inches away from the subject X and O are not
recognizable.
Diagnosis: The patient’s Right eye recognition of X and O is Normal.

2. Assessment of Extraoccular Muscles (EOM)

Extraoccular Muscles (EOM) in the Left Eye
Direction Complied to Follow (CF) or Not
Complied to Follow (NCF)
Medial CF
Lateral CF
SR CF
IR CF
SO CF
IO CF
Diagnosis: The patient’s Left eye Extraoccular Muscles (EOM) is Normal.

Extraoccular Muscles (EOM) in the Right Eye

Direction Recognizable or Not Recognizable

Medial CF
Lateral CF
SR CF
IR CF
SO NCF
IO CF
Diagnosis: The patient’s Right eye Extraoccular Muscles (EOM) is Normal.
3. Recognition of the Six Consecutive Numbers in the Lateral Position while the eyes (of
the subject) were at the Medial Position.

Left Eye Right Eye

1 1 1 0 1 1
Diagnosis: Recognition of the patient on the Six Consecutive Numbers in the Lateral Position is
slightly affected. Affected area is the right eye.

 Skin/Touch Assessment
1. Recognition of different objects with different texture
Object Recognized or Not Recognized
Balled end of Pen (flat) Not Recognized
Pointed End of Ball Pen (pointed) Recognized
Cotton (soft) Recognized
Ballpen (hard) Recognized
Diagnosis: The tactile recognition of different objects with different texture of the patient is
slightly affected.

 Nose/Smell Assessment
1. Recognition of the alcohol sprayed in the air (Both Eyes Closed)
 Rosanna successfully recognized the smell, 22 seconds after the alcohol was sprayed in the air.
Diagnosis: The olfaction of the patient while both eyes are closed is Normal.

2. Recognition of Different Spices

Spice Recognized or Not Recognized

Rose Mary Recognized
White Pepper Not Recognized
Basil Leaves Not Recognized
Chili Powder Not Recognized
Diagnosis: The olfaction of the patient in recognizing different spices is Not Intact.

 Tounge/Taste Assessment
1. Recognition of different Food with both eyes closed

Food to Taste Recognized or Not Recognized

Apple Not Recognized
Prunes Not Recognized
Cheese Not Recognized
Banana Recognized
Carrots Recognized
Sugar Recognized
Egg Recognized
Diagnosis: The gustatory functions of the patient in recognizing different Food with both eyes
closed is Not Intact.

 Ear/Hearing Assessment
1. Rinne’s Test

 The Rinne test done to Rosanna shows that air conduction (AC) is greater than bone conduction (BC) in
both ears.

2. Weber’s Test
 The Weber test done to Rosanna lateralizes stronger to her left ear.

Diagnosis: Then there is sensorineural hearing loss in the opposite (weaker) ear (the right ear).

 Swaying Assessment
1. Swaying Assessment

Position Swaying of the Subject

Anatomical Position (Eyes Open) Normal
Anatomical Position (Eyes Closed) Forward, Right
Arms Up (Eyes Open) Forward, Right
Arms Up (Eyes Closed) Forward, Right
Right Leg Up (Eyes Closed) Forward
Left Leg Up (Eyes Closed) Forward

2. Swaying after turning around 10 times and the subject will pick up three coins

 Rosanna swayed to her right after being turned around 10 times.

Diagnosis: The patient’s affected side is the right.

 Reflex Assessment
Reflex Normal or Not Normal
Knee Normal
Feet/Toes Normal
Diagnosis: The patient’s reflexes on both the knees and the toes are Normal.
