AFFIDAVIT OF LOSS

I, FELISA RAYOS-CATILO, of legal age, Filipino,

and a resident of Sampaguita, Lipa City, Batangas, after
having been duly sworn to in accordance with law, hereby
depose and state that:

[1] I am the registered owner of a parcel of land,

situated at Tinga Labak, Batangas City, and covered by
Transfer Certificate of Title (TCT) with No. T-40747;

[2] I have entrusted the possession of the original

copy of said TCT to my daughter, HAZEL C. BAJA;

[3] Recently, it was discovered that said TCT was

missing;

[4] I exerted diligent efforts to find and locate said

TCT but the same proved futile;

[5] I am executing this affidavit to attest to the truth

of all the foregoing facts and to inform the proper
authorities particularly the Register of Deeds of Batangas
City as regards the loss of the original TCT and for
whatever legal purpose this may serve.
