Y in and yang, the gonads include female
ovaries and male testes.Some gonadal diseases
are similar or identical whether occurring in the
testes or the ovaries (eg, sex cord stromal and germ
cell tumors [GCTs]); others are unique to the specific
organ.Fine needle aspiration (FNA) biopsy of the male
genital system (exclusive of prostate) and the female
pelvis also are discussed in this chapter.

In practice, gonadal sex cord tumors, such as Sertoli tu-
mors and granulosa cell tumors, may be difficult to distinguish
from one another, as well as from Brenner tumors, by cytology
alone.
Ovarian Fibroma/Thecoma
Ovarian fibroma/thecoma |i20.3| usually has a low cell
yield on FNA biopsy. The tumor is composed of spindle cells
|i20.4|. Fibroma cells are thin and spindly. Thecoma cells are
Fibrothecoma (P) plumper spindle cells with more irregular nuclei and more abun-
dant, fragile, lipid vacuolated cytoplasm. The cervicovaginal Pap
test may show increased maturation in postmenopausal patients
owing to estrogen production by the tumor.The differential diag-
nosis includes leiomyoma.

Embryonal Carcinoma
The FNA biopsy specimen of embryonal carcinoma is
cellular and composed of poorly differentiated epithelial-like cells

also may be seen. The cytoplasm is delicate with indistinct cell
borders (syncytial growth pattern).The nuclei are large, irregular,
and pleomorphic, with coarse chromatin and prominent irregular

Choriocarcinoma is rare, particularly when pure, but
occurs somewhat more often as a component of other GCTs.

Teratoma
Teratomas are composed of cells from all three germ
cell layers (endoderm, mesoderm, and ectoderm).
Mature Cystic Teratoma: Mature cystic teratoma is the
most highly differentiated GCT; it is benign. Mature teratomas are
predominantly cystic and can grow very large. Calcification is
common.The tumor is filled with granular keratinous debris.The
FNA biopsy may obtain skin and its appendages (eg, squamous
cells, squames, sebaceous cells, hair) |i20.20| and possibly neural,
gastrointestinal (including pancreas), cartilage, and ciliated respira-

are present, by definition. In men, the differential diagnosis includes
columnar cells and mucus aspirated from a scrotal hernia; in
women, it includes squamous contaminants aspirated transvagi-
nally; and in either sex, normal colorectal mucosa, transitional
epithelium, mesothelium, or soft tissue is included.
Immature Teratoma: Immature teratoma is usually a solid
tumor but can be cystic and is characterized by the presence of
immature or malignant cells: epithelial (eg, squamous or adeno-
carcinoma), mesenchymal (eg, chondrosarcoma or osteosar-

Teratocarcinoma: Teratocarcinoma is the combination of
embryonal carcinoma with mature or immature teratoma.

Endometrioid Carcinoma
Endometrioid carcinoma resembles serous adenocarci-
noma but has scanty, more granular cytoplasm, nuclear crowding,
and microacini |i20.31|, |i20.32|. Squamous differentiation can
occur.A mucoid background and hemosiderin-laden macrophages
may be seen.The presence of papillae favors a serous tumor.The
presence of gross mucoid material favors a mucinous tumor. Endometrioid carcinoma (DQ)

Miscellaneous Tumors
Undifferentiated carcinoma is characterized by anaplastic
malignant tumor cells.Intracytoplasmic hyaline globules may be seen.
Small cell carcinoma is a rare aggressive tumor often
Clear cell carcinoma (P) associated with hypercalcemia.The tumor may be composed of

small tumor cells and large cells resembling luteinized cells.
