Provision of Tubular Inspection Services in Block 60. HSSE
LEVEL - 2
1.1 All defined terms in this Exhibit F shall have the same meaning as set out in the
Contract unless otherwise stated.
1.2 Any breach of the HSSE Specifications established in Exhibit - F shall be deemed by
the Company to be a material breach of the terms of the Contract between the
parties and the Company shall be entitled to take appropriate action including
instructing the Contractor to (a) remedy the breach; (b) suspend Work / Service or (c)
terminate the Contract.
1.3 Company reserves the right at all times to audit and review Contractor’s facilities,
services and/or performance of its activities in respect of compliance with the
accepted Contract HSSE Plan for the Work / Service.
1.4 Company reserves the right to suspend the Work / Service or any part thereof if
Contractor does not comply with the accepted Contract HSSE Plan at the risk of the
Contractor. Before any Work / Service is suspended Company shall provide notice
with Contractor to allow Contractor the opportunity to rectify any non-conformances
within an acceptable timescale acceptable to the Company. Any suspension shall be
carried out in accordance with the terms of the Contract.
1.5 Either party may, at any time, suspend the Work / Service for HSSE issues; in such
event where the Contractor elects to suspend the Work / Service, Contractor shall
immediately inform Company in writing of those reasons, and provide details of
actions taken to mitigate, reduce, or eliminate the issues.
1.6 It is expected that the Contractor shall have its own HSSE standards and/or policies
which apply to HSSE matters and shall comply with them where applicable. Where
these standards are not available, or in the reasonable opinion of the Company, they
fall below Company’s own standards then the Company shall have the right to issue
its own standards to the Contractor, in order that the Contractor shall comply with
these provisions.
1.7 Failure by Contractor to adhere to, demonstrate compliance with, or ensure
Contractor Personnel complies with the Company’s HSSE Exhibit or Standards may
result in termination of the Contract as specified above.
1.8 All Contractor reports and documents for the management of HSSE activities under
this Contract shall be made available to Company in the English language.
2.1 Contractor shall ensure that all Contractors’ Personnel are briefed, understand and
strictly adhere to the provisions of this Exhibit F all relevant Standards, Rules or
Guidelines for HSSE.
2.2 Contractor’s management shall demonstrate leadership and commitment through
actively participating in all aspects of HSSE, supporting open dialogue and by
allocating sufficient and competent resources to the Contract.
2.3 Where applicable Contractor’s management shall exercise a ‘no blame’ policy to
encourage employees and empower them to ‘Stop the Job’ to prevent unsafe
actions, and enable intervention to reinforce good behaviours.
2.4 Contractor shall ensure that HSSE responsibilities, authorities, accountabilities and
competencies in relation to the Work / Service are clearly defined, documented,
communicated and exercised at all levels.
2.5 Contractor shall set clear Contract HSSE goals, objectives and targets and ensure
that performance is evaluated against them.
2.6 Contractor shall formally review HSSE performance in relation to the Work / Service
at regular and frequent intervals to ensure that Contract HSSE goals, objectives and
targets are being met and areas of concern are addressed.
Guidelines (Report No 423; June 2010) and the Contract HSSE Plan must address
the HSSE Hazards and associated risks and the statutory/regulatory obligations
specific to the Scope of Work set out in the Contract. It should also detail the
management controls that will be implemented during the course of the Work /
Services to eliminate reduce or mitigate the risks so as to ensure full compliance with
all regulatory requirements. The Contract HSSE Plan will also reference other
specific Contractor HSSE document associated with Contract activities (e.g.
Mobilisation Plans, Commuting Plans, Lifting Plans, and Method Statements, etc.
etc.) Company reserves the right to request additional HSSE documents or plans as
may be required.
5.3 The Contract HSSE Plan shall contain a Hazard ID / Risk Register which addresses
the Contract HSSE risks and requirements of all phases of the Work / Service
through mobilisation, execution and demobilisation at each location where the Work /
Service will be performed (including but not limited to the office, work place,
fabrication yard, lay down area, construction site, vessel, offshore installations, waste
and wastewater management facilities) known as the Worksite. The HSSE Plan
shall demonstrate how risks to all personnel and the wider environment have been
identified and have been reduced to as low as reasonably practicable (ALARP).
1.1 The Contract HSSE Plan shall contain the list of HSSE deliverables along with a schedule for
their completion. These deliverables will include:
HSSE goals, objectives and targets for the Work / Service
A detailed project organisation chart outlining Contractor’s HSSE management
structure for the Work / Service
A schedule of regular leadership/management visits and/or inspections
designed to ensure leadership involvement and accountability
A list of method statements and associated procedures that will be prepared
and/or used during the course of the Work / Service to manage hazardous
activities
Detail the PPE requirements for the Work / Services, including any specific
and\or specialist requirements
A schedule of HSSE Inspections, audits and reviews that will be carried out to
measure the HSSE performance of the Work / Service
A formal training matrix detailing the minimum competency requirements for
each position or activity – To be updated and presented to OOCEP on a
monthly basis.
5.4 The Contract HSSE Plan shall comply with Company recognised international
association of Oil & Gas Producers (OGP) Life Saving Rule (LSR) requirements.
5.5 The Contract HSSE Plan shall be submitted to Company for review and acceptance
within thirty (30) days from the Effective Date of the Contract or before the
mobilisation of the Contractor to the Worksite (whichever is the sooner). The plan
shall be revised and if necessary updated annually (as a minimum).
5.6 The Contract HSSE Plan will also contain reference to Contractor mobilisation and
commuting plans.
6.0 HAZARD IDENTIFICATION, EVALUATION AND RISK MANAGEMENT
6.1 Contractor shall ensure that all Hazards have been identified, for its Work / Service,
and that a documented risk assessment procedure and a risk register are in place
and operating. This risk assessment procedure shall be suitable and sufficient to
appropriately assess the health, safety, security and environmental risks involved. A
copy shall be attached to the Contract HSSE Plan for review by the Company.
6.2 Contractor shall be responsible for ensuring timely completion of the risk assessment
for activities, covered in the Scope of Work, in order to meet the Work schedule, the
Company HSSE MS and regulatory requirements.
6.3 Contractor shall give Company written notice within a reasonable timescale before
the delivery or removal from the Worksite of any chemical or substance which is toxic
or hazardous to health or potentially harmful to the environment. The notice shall
identify the hazards and effects and assess the risks to personnel and the
environment. Details of the precautions to be taken when using, handling,
transporting, storing or any other means of contact will also be provided in the form
of a “Material Safety Data Sheets” (MSDS).
6.4 Contractor shall ensure that at all times that any potentially harmful chemical or
substance is suitably packaged and labelled and has been assessed in accordance
with the requirements of Omani regulations and applicable laws on the use,
packaging, storage, registration and transportation of Chemicals and Hazardous
Materials (RD 46/1995, MD 249/1997, MD 317/2001), the Control of Substances
Hazardous to Health (COSHH Regulations), published by the UK Health and Safety
Executive. (In Oman the PDO Chemical Management Manual can be used as
reference.)
6.5 Contractor shall ensure that all workers are provided with all necessary Personal
Protective Equipment (PPE) at the Worksite. This shall, as a minimum, meet
Company and recognized international standards.
6.6 Contractor shall conduct Risk Assessments for all activities requiring the use of PPE,
the specification of the required PPE, and provide additional and/or specialist PPE
as/when required.
6.7 Contractor shall ensure that the PPE issued to the employees meet the requirements
of the Risk Assessment for the Work / Service activity and will ensure that the
contents of the Work / Service Risk Assessments are communicated to the workforce
during the pre-job Tool-Box-Talks (TBT) or safety briefings.
6.8 Contractor Personnel failing to use the appropriate PPE will be asked to use the
appropriate equipment or stop Work / Service immediately. If the Contractor
Personnel fail to use the PPE the Company may required them to stop Work /
Service and/or leave the Worksite immediately. Persistent repeated failures by
Contractor Personnel to use appropriate PPE may result in Company requiring the
Contractor to permanently remove the relevant Contractor Personnel from the
Worksite and replace the Contractor Personnel without delay and at no extra cost to
the Company.
7.1 The Company has adopted the international association of Oil & Gas Producers
(OGP) Life Saving Rules as an internationally recognized best practice and require
all contractors to apply the LSR requirements when carrying out the Work / Service.
7.2 Contractor shall establish LSR training events and maintain records of all Contractor
Personnel and all Subcontractor Personnel LSR training
7.3 Contractors are also required to adopt consequence management practices aligned
to those of the Company for any LSR breaches/failures.
7.4 Records of all LSR related incidents are to be maintained by the Contractor.
8.1 Prior to mobilisation, Contractor shall provide the Company with a detailed Contract
HSSE Mobilisation Plan. This plan will list all vehicles and equipment intended for
use on the Work / Service; along with evidence or vehicle/equipment licences,
permits, certification and insurance. The Company reserves the right to inspect all
vehicles and equipments and documentation prior to mobilisation to Worksite.
8.2 Contractor shall provide written HSSE Method Statements and Procedures to cover
Work / Service related hazardous operations. These will be available to all
Contractor’s Personnel and Subcontractors in their working language. A copy shall
be provided to Company for review prior to Work / Service commencement.
8.3 Contractor shall abide by either the Company Permit To Work (PTW) system or a
Company approved PTW system when carrying out Work / Service at the Worksite.
8.4 Contractor shall apply Job Safety Analysis (JSA) commensurate with the nature of
the Work / Services, the disciplines and interfaces involved, and the Worksite
conditions and environment. This shall apply to all Contractor and major
Subcontractor work activities including work interfaces and all collaborations with
Company and Company’s other major contractors. Work procedures including
applicable JSAs shall be communicated to all personnel involved and impacted by
the work on each occasion the work is repeated or new personnel become involved.
8.5 Contractor shall have a process for assessing the effectiveness of JSAs and
implementing the findings to improve the quality of the assessments.
8.6 Contractor shall provide a written procedure for obtaining Company’s permission to
deviate from HSSE Policies Standards or Guidelines. This will be available to all
Contractor’s Personnel and Subcontractors in their working language. A copy shall
be provided to Company for approval.
8.7 Contractor shall establish, adopt and implement emergency procedures related to
the Work / Service. Contractor shall consult with Company to ensure appropriate
interfaces with Company Emergency Response & Crisis Management procedures.
8.11 Copies of the proposed Commuting Plans must be submitted for approval by the
Company before mobilisation of any Contractor Personnel to the Worksite.
8.12 The Contractor shall identify and obtain all necessary permits, consents and licenses
from any government authority, municipality or any other statutory authority for the
performance of the Work / Service. The Contractor shall comply with all conditions or
restrictions imposed by any such permit, approval or licence.
8.13 The Contractor shall submit a Permit Plan to the Company for information within 30
days of the Effective Date. The Permit Plan shall list:
All required permits, approvals and licenses
The authority to which application must be made
The planned date for obtaining the consent taking into account the expected
time for the authority to consider the application for consent
9.1 Repeated failure by Contractor to comply with the Contract HSSE requirements may
result in the request by the Company of the removal from Worksite of the relevant
Contractor’s supervisory and/or managerial Personnel.
9.2 In the event that Company considers that Contractor has failed to comply with the
latest approved Contract HSSE Plan, or other Contract HSSE requirements, then
Company shall issue Contractor with an HSSE Non Conformance Report (NCR) .
9.3 For the purposes of this Non Conformance Report clause:
