US Consular Interview so please take note of the following important STEPS relating to your
EB3 Visa application.

STEP1: 02/07/2019 – Medical Exam at St. Luke’s Medical Extension

Guardian Healthcare through Fil-Overseas will be sponsoring the following (to be provided
to you before or during the Pre-Interview briefing):

2. Taxi fare to and from US Embassy

Carefully read the attached Consular Interview FAQs.

Kindly ensure that you have the following US Embassy required documents and bring
them to the Pre-US Embassy Interview Briefing so we can check them:
BASIC REQUIREMENTS:

o Consular Appointment Letter

o DS 260 Confirmation Page (Confirmation Page from Guardian c/o FOESI to be given
during your Pre-US Embassy briefing)

o Original Passport/s (valid for at least six (6) months from US Consular Interview date)
and all expired passports (if available) for all traveling applicant/s.

o 3 pcs 2x2 size photo (white background) for each applicant

o Official Job Offer (Consular Letter from Guardian c/o FOESI to be given during your
Pre-US Embassy briefing) with the salary stated from the potential U.S. Employer, issued
less than one year prior to visa application

SUPPORTING DOCUMENTS:

o Approval Notice (NOA)

o Original PSA (Philippine Statistics Authority) Birth Certificate

o Original PSA (Philippine Statistics Authority) Marriage Certificate (If Applicable)

o New Original NBI Clearance (for travel to USA valid for at least 6 months at the time
of your consular interview, ALSO, if you have your name changed with an annotation on
your PSA (Philippine Statistics Authority) Birth Certificate, please secure an NBI
Clearance for both names)

o Original Police Clearance (from any country outside the Philippines that applicant
resided for more than six (6) months)

o Original Visa Screen Certificate (from Guardian c/o FOESI to be given during your
Pre-US Embassy briefing)

o Old and Current Professional ID’s (if available)

o Original or photocopy of NCLEX License and Result (if applicable) (from Guardian
c/o FOESI to be given during your Pre-US Embassy briefing)

o Original PSA (Philippine Statistics Authority) Birth Certificates of dependents


o Original or certified copy of Military Record (if applicable)

o Original Certificate of No Marriage CENOMAR (if applicant was never been married)

We are aware that all of you have submitted the original copies of the above listed documents
together with your DS260. However, because of the notation on your Consular Appointment
Letter which states “Please note that even though the applicant may have provided the NVC
with a copy of a document, the applicant is still required to bring the original document to
the visa interview.” it is best that you prepare these documents to avoid unnecessary delays if
ever the Consul will require.

back to FOESI with X/s on all the appropriate line/s below:

1. REGARDING DRIVER’S MANUAL - Please make sure that you STUDY the manual
sent to you by Ms. Lady Obedos and practice driving, and that you are not only knowledgeable
in driving but that you will pass the written and road test of the Department of Motor Vehicle in
Tennessee upon your arrival.

2. UPDATED RESUME (GHP Format) – Please send (if you have not done so) your
updated resume in GHP format to Guardian cc Jerald ASAP.
