APPPLICATION FOR PERMIT AND ACCEPTANCE OF CONDITIONS

1. I, theundersigned, hereby make application, pursuant to the provisions of Act No. 3077 and Bureau of Lands
Administrative Order No. 8, amended for extension to temporarily or provisionally occupy all use for
BATHING ESTABLISHMENT purposes the tract of land located at Barrio Antipuluan, Municipality of Narra,
and Province of Palawan containing approximately an area of 690 sqm. more or less ; bounded as follows:

S: FORESHORE LAND W. SALVAGE ZONE
(If the lands correspond to a lot in a cadaster or subdivision, mention the lot number as
well as the number of the cadastre or subdivision)

The land is covered by my Revocable Permit Application No. 045315 - ___; I shall use the land for
BATHING ESTABLISHMENT.

2. I make this application for my exclusive benefits and not for the benefit of any person; and I shall not lease
or sublease the land applied for and/or the improvement thereon to any other person nor shall allow such
other person to derived benefits there from either directly or indirectly.
3. To show that I possess the qualifications required by law, the following statements of fact is submitted.
4. My name is VICTORIANO M. MANABAT; age is 73 years; I was born in April 22, 1945 and employed in
SELF- EMPLOYED. I am citizen of Philippines; I am married/single and the name of my wife/husband is
LOLITA S. MANABAT; who is the citizen of the Philippines.
5. I am not holder of any other application for public land. These are follows: n/a
6. I have read the instruction and conditions applied in this application and hereby undertake to abide by
themas well as the provision of the aforesaid Administrative Order No. 8, as amended, under which the
permit is to be issued, and I promise to comply with all the laws and regulation governing the occupation
and use the public lands. THIS APPLICATION MAY THEREFORE BE CONSIDERED AS MY ACCEPTANCE OF THE
CONDITIONS PRESCRIBED BY EXISTIING LAW RULES AND REGULATIONS AS WELL AS THOSE OTHERS WHICH
TO PROTECT PUBLIC INTEREST, MAY BE PROPOSED BY THE SECRETARY OF ENVIRONMENT AND NATURAL
RESOURCES OR THE DIRECTOR OF LANDS. AND EXPRESSLY STATES ON THE BACK HEREOF AND IN THE
PERMIT TO BE ISSUED.

CONDITIONS UNDER WHICH PERMITS ARE ISSUED

1. This application is not made for any benefit of any person other than the applicant.
2. This permit is to be issued under the condition prescribed by Administrative Order No. 8, as
amended and each additional conditions as the Secretary of Agriculture and natural resources of the
Director of Lands may deem necessary, according to attending circumstances or to protect public
interests, which special conditions shall be expressly stated in, and made a part of, the permit.
3. The permittee shall occupy and put the land subject of the permit to the use for which the
permit is to be issued within two (2) months from the date of the permit, upon accompanied by the
forfeiture of the fees paid for failure to occupy and use the land within the period herein allowed.
4. No building or structure shall be constructed or erected on the land without its plan and
construction having been previously approved by the corresponding authorities in accordance with
local ordinance.
5. That the permit is not transferrable and neither the land nor the improvements thereon
shall be leased, transferred, sold, conveyed, mortgaged, or in any manner encumbered or assigned
without written consent of the Secretary of Agriculture and Natural Resources, the Under Secretary of
the Director of Land, as the case may be which consent may be given only 1. AND WHEN THE SAID
PERMIT IS STILL SUBSITING OR WHEN THE PERIOD HEREOF HAS NOT YET EXPIRED, and after it has been
found that the said transfer has not been made for the purpose of speculation and that all permit fees
have been paid up to date of approval. IN THE EVENT THAT THE PERMIT HAS ALREADY EXPIRED? NO
TRANSFER OR ASSIGNMENT SHALL BE ALLOWED BUT THE APPLICATION SHALL ONLY BE RECORDED IN
THE NAME OF THE NEW APPLICANT WHO DESIRES TO SUCCEED THE ORIGINAL PERMITTEE. In any case
of transfer or assignment, the transferor and the transferee shall be held jointly and severally liable for
any arrears in the payment of the permit fees by the transferor.
6. Should be the permit be cancelled by reason of the failure of the permittee to any the permit
fees, all improvements found thereon shall be forfeited to the Government.
7. Upon discovery, the permittee shall immediately notify in writing the Director of Lands of the
existence in the land of any gold, silver, copper, other substances containing minerals, guano, precious
stones, coal or coal oil, and he shall not take or remove nor suffer others to take nor remove from the
place any auntity of the said product.
8. The permit is subject to the favorable recommendations of District Engineer the
Commissioner of Customs, and further findings of the District and Officer concerned, if said
recommendation and findings are necessary but were not available and were therefore not considered
at the time for the issuance of the permit.
9. Persons holding licenses issued by the Bureau of Forestry shall have the right – of – way on
the land and also the right to cut, collect and remove timber thereon.
10. Any timber growing on the land shall not be cut and dispose of for commercial purposes
without license from the Bureau of Forestry.
11. No, improvements found on the land covered by the permit shall be sold, mortgaged or any
manner encumbered without the written permission of the Director of Lands.
