SUPPLEMENTAL COMPLAINT–AFFIDAVIT

I, Mr. XXXXXXXXXXXXXXXX, Filipino, of legal age, single and

with address at Purok 69, Brgy. Pantalon, Mang Tomas City, do hereby charge

XXXXXXXXXXXXXX, Filipino, of legal age and with residence at

Purok 69, Brgy. Pantalon, Mang Tomas City, Philippines,

XXXXXXXXXXXXXXXXXXXXX, Filipino, of legal age and with

residence at Purok 69, Brgy. Pantalon, Mang Tomas City, Philippines,
- And -

XXXXXXXXXXXXXXXXXX, Filipino, of legal age and with

residence at Purok 69, Brgy. Pantalon, Mang Tomas, Philippines,

with violation of Article 282, paragraph 1 of the Revised Penal Code for
Grave Threats and/or violation of Article 358 of the Revised Penal Code
and/or such other crimes as may be deemed warranted under the circumstances,
and for said purpose, after having duly sworn to in accordance with law do
hereby depose and state that:

1. I am the Private Complainant in this particular case who may be served

with pleadings, orders and resolutions of the court at Purok 69, Brgy.
Pantalon, Mang Tomas, Philippines;

2. I am filing this supplemental complaint-affidavit to supplement or amend

the paragraph 5 of my original Complaint-Affidavit executed before the
public prosecutor. The original Complaint-Affidavit states, that:

3. The supplement or amendment seeks to clarify the involvement of

xxxxxxxxxxxxxxxxxxxx which was not specifically included in the
original Complaint-Affidavit. The aforesaid supplement or amendment
states, to wit:

4. Consequently, the said unlawful act of threatening another of the

infliction of the crime is punishable under the law in violation of
Article 282, Paragraph 1 for GRAVE THREATS under the
Revised Penal Code of the Philippines, to wit:

“ ART. 282. Grave threats. - Any person who shall

threaten another with the infliction upon the person, honor,
or property of the latter or of his family of any wrong
amounting to a crime, shall suffer:

1. The penalty next lower in degree than that

prescribed by law for the crime he threatened to commit, if
the offender shall have made the threat demanding money or
imposing any other condition, even though not unlawful,
and said offender shall have attained his purpose, the penalty
lower by two degrees shall be imposed. x x x “

10. Also, the said unlawful act by XXXXXXXXXXX and

XXXXXXXXXXXXX by shouting against me defamatory words in front
of the public is punishable under the law in violation of Article 358 for
GRAVE ORAL DEFAMATION under the Revised Penal Code of the
Philippines, to wit:

“ ART. 358. Slander. - Oral defamation shall be punished by

arresto mayor in its maximum period to prision correccional in its
minimum period if it is of a serious and insulting nature: x x x “

13. I am, therefore, executing this affidavit to attest to the truth of the
foregoing facts and for the purpose of charging XXXXXXXXXXXX,
XXXXXXXXXXXXXXXXXX, XXXXXXXXXXXXXXXXXXXXXXX
for violation of the provisions of Article 282, paragraph 1 of the Revised
Penal Code for Grave Threats and/or for violation of the provisions of
Article 358 for Grave Oral Defamation of the Revised Penal Code
and/or such other crimes as may be deemed warranted under the
circumstances, and for said purpose.
