ULC-47. XEN/Pway has advised of imposition of TSR of 55 KMPH in between DM-DW

between mast no. 21/09 - 21/21 at UP line, in between DM-DW in Line-03, TSR of 50 0 #N/A

Digital clock displaced (hanging) at PF no-02, CSS-T11.As per CSS.Digital clock reset

At 6:23 hrs at Yamuna bank depot, Point 318W flashed. Induction of trains towards

At 8:12 Hrs at SN Dn Point Zone,TC 313G dropped. Train 330 set# B21 moved in
ROS upto SN Dn, following 12 more trains also moved in ROS. P way after
footplating requested 5 min ATT for tightening the loose clamp of jumper placed at
the Rail in effected TC. At 9:03 Hrs ATT was granted to JE P way along with 2 staff for
tightening the jumper clamp. The ATT was cancelled at 9:11 Hrs. During failure
trains were regulated, centralized announcement done in section from DW to IPDT SPL #N/A
and in trains hold up in the section.
Repercussion: i) One round tri[p each DW-VASI and DWTO- NCC cancelled.
ii) One round trip DW-NCC delayed.
PTM line below pump room at lift-3 pannel hole is damaged condition. As per TPC,

JE/Pway, XEN/Pway had requested to impose TSR of 30 KMPH for train approaching
YB PF-01 between OHE mast no. SE/8-SE/12, i.e. between TC331P-TC333P to 0 #N/A

At 18:59 hrs at UNW UP; Unusal sound & abnormal mark observed in track near 8
car NSP by SC. TPC & P-way staff apprised. As per TPC, P-way staff checked & found
Fish-plate in loose condition but no abnormality in train movement. 0 #N/A
At 12:00 hrs; at RN/DN, a passenger (age-70 yrs approx) jumped/ fell down in front
of Train no.330 (TS#21). Train Operator applied EB but first two coach of train
crossed over and passenger came under third coach of the train. Train movement
stopped immediately at both lines and PCR/CATS was called. SC/RN was instructed
to reach the incident site and rescue the passenger. Train was evacuated by SC/RN
(Approx. 500 passengers) and train sent to YBD. The injured passenger was rescued
by station staff and handed over to PCR (PCR-13, Constable, Mr Sanjiv & Mr Rajesh).
To minimise bunching of trains, short looped operation was initiated KNR-NECC/VASI
(2 Trains) SN- DWTO (1 Train), JPW_DWTO (01) & YB-NECC/VASI (3 Trains). During
failure, centralised announcement of delay citing reason was done at stations and in
trains. All concerned apprised. Details of passenger awaited.

in RM/SCS mode from S601D to STVR DN pf NSP during failure. At 12:55 hrs at STVR,
ATT was granted to SE/Signalling along with 04 members for maintenance of TC 615D
between STVR-JLA in DN line. During ATT train regulation in DN line ensured
accordingly by TC/OCC & cautious movement in UP line trains ensured with caution
order from STVR UP pf by SC. ATT was cancelled at 13:07 hrs & normal train
movement resumed. In night P.Way staff attended but fault not rectified. On advise of
P.way, Point 603D clamp & padlock in normal direction.

As per FMC-PTW by EPIC was taken for installation of second drive of point P603D at
STVR not for pad lock the point. After second drive installation point was declared unfit
for reverse movement jointly by P-way and S&T. This note was given to STVR/SC, after
which STVR/SC clamped pad locked the point in consultation with OCC. EPIC of S&T/ P-
way does not have any role in pad locking the point. Pad-locking of point is
responsibility of station controller. So failure should be on account of operation.
TC 620 P dropped, intermittently. Target Speed reduced to Zero for trains departing
from BVHM UP pf. All trains moving in RM/SCS mode from TC620 P to NCBC/ UP pf
during failure. Total 10 trains moving in RM/SCS mode.Accumulated repercussion:-

the door again for platform service. Passengers verbally complained at SCR regarding

and found OK. Brake demand 1 was varying at the time of Failure. In
depot all BECU status was OK. Further this failure will be under

At 00:02 hrs, T.O. of T.No-7310#26 approching JLA dn pf reported that
one person was on track and he stopped his train by applying EB. The
train stopped in midway and T.O. again reported that person came
underneath the train. OVA station controller along with CISF security staff
sent immediately to location. T.O. made delay announcement for the
passengers inside the train. SC/OVA & CISF staff checked the section and
train underframe but no person was found. At 00:22 hrs, SC/OVA given
track clearance to T.O. then train moved at walking speed to JLA dn pf.
SC/OVA reported that 02 nos. of cable having length 20 mtrs each found
SPL ###
cut and one cutter was found near mast no-29/35. TPC & FMC apprised
to check the section & instruct the concerned department to lodge the
FIR. The cutter was brought to SCR/OVA. SC/OVA reported the incident to
DMRP. Later TPC informed that section was checked in night by PSI staff
& one 20 mtrs cable of 33 kv found stolen.

BECU major fault & BECU status faulty in all motor car showing when
mode selector at ATO mode. In ATP mode train departing normally all
fault cleared itself. AS per RSC:-TCMS data checked and found that "BECU
Fault- Brake Demand1&2 RANGE FAILURE , BECU Fault- RED LAD03,
BECU Fault - Major/Minor event status, BECU Major Event 1 &2, CI Fault -
PWM signal fault of PWM Generator (1) " logged in all motor cars at
19:48:15/JPW station. ER data checked and found that After Cab
changing, when DM-2 Mode selector moved to ATO position at 19:48:15

and PWMSR become high. At that time all above failure logged and not
cleared even after MS mode changed to ATP(2 times) . From JPW to
BTGD, train was running in TCMS back up mode only as failure reset itself
after cab changed at 20:53:42. BECU data checked and found same. In
depot, Wiring for PWM supply & Brake demand wiring VATC to PWM
checked for looseness and found normal. MSATOR 1 contact(10-6)
resistance checked and found normal . Further on suspected basis,
MSATOR1 relay has been replaced from spare.
PAD operated, TO failed to talk with passenger after that multiple PAD
operated. As per RSC,In depot Checked . AVAU event log and observed
that DM2 PAD2 was activated at10:02:51 it was not acknowledge and
between this only manual PA announcement was done several times

On 11-Apr-2019:- Train running normal, no failure reported on M/L, In depot HMI data checked and found
