and say:

2. That I am a member of the Home Development Mutual Fund with Pag-
ibig Number _____________;
3. That in my Pag-ibig records, what appeared in my full name was
__________ and there was no indicated middle name thereat;
4. That in my Philippine Statistics Authority Certificate of Live Birth and
all other public documents, my correct given name, middle name and
last name was correctly indicated;
5. That such discrepancy in my Pag-Ibig records was merely clerical
during the time of the registration;
6. That both names pertain to one and the same person as referring to
me; and
7. I am executing this Affidavit to attest the truth of the foregoing
specifically for my documentary requirements for Pag-Ibig purposes
and for whatever legal purposes it may serve.
