A. Purpose/Aim
The purpose of this safe work procedure (SWP) is to define the proper safety
measures that have to be taken while carrying out the individual work activity
during Post-tensioning works. The aim of procedures is to prevent
incidents/accidents from happening to the personnel involved in the work
activity.
B. Scope
This safe work procedure (SWP) shall cover the procedures of the 31 work
activities identified in the Risk Assessment for Post-tensioning Works, which is
applicable for general Post-tensioning Works.
C. Objective
The objective of this procedure is to address the risk control measures and set
safety guidelines to the work methods for all workman and personnel involved
in the activity. As such, it is targeted to minimise the possibility of
accident/incident occurring during the work activities.
The following are the work activities identified during the Risk Assessment for
General Post-tensioning Works.

B. Roles and Responsibilities
The Site Supervisor and Safety Supervisor are to ensure that the procedures
in the SWP are informed and explain to the personnel involved in the works. It
shall be the responsibility for all personnel involved in the works to follow the
procedures within practical means as close as possible to ensure that all
works are carried out in a safe manner.
If the conditions of the work area are not safe to carry out the work
procedures, work must be stop and the Main Contractor shall be informed for
rectifications works.
All PPI employees are issued with one full set of Personal Protection
Equipment and a backpack, which they are to carry with them at all times to
the projects assigned to them. The lists of standard PPE are as follows:
The Site Supervisor shall acquire any additional PPE he deemed necessary to
better protect the personnel involved in the activities.
The Site Manager, Site Supervisor and Safety Supervisor are to conduct
monthly check on the PPE issue to the work personnel. Prior to the actual
works, the Site Supervisor and Safety Supervisor are to ensure that the PPE
worn by the work personnel is in good conditions. It is the responsibility of
individual users to check their own PPE every time before they use.
D. Training
All PPI personnel are required by the PPI Management to attend PPI Induction
Training prior his employment. Toolbox Meeting is to be conducted daily
before the start of any operation.

For activities identified with driver’s unaware of in – house rules, the following
procedures are to be carried out:
Project In Charge and Site Supervisor to ensure bank man to be present
at all times to guide the vehicle.

following procedures are to be carried out:
Project In Charge and Site Supervisor to ensure that only certified
material cages used for hoisting of material.
Project In Charge and Site Supervisor to ensure certified ducting frame
to be used for hoisting of ducting.
Project In Charge and Site Supervisor to ensure that the material cages
and ducting cages are not fully loaded to prevent spillage of material.
For activities identified with lifting gear give way, the following procedures are
to be carried out:
Project In Charge and Site Supervisor to ensure only certified and valid
lifting gears to be used.
Project In Charge and Site Supervisor to check for validity of the lifting
gears and damages of the lifting gears before used.

Project in Charge and Site Supervisor to check for validity of the lifting
gears and damages of the lifting gears before used.
For activities identified with hoisted material/ equipment hitting onto other
properties or worker on site, the following procedures are to be carried out:
Project in Charge and Site Supervisor to ensure there is proper
communication between the signalman and the crane operator at all
times during the hoisting.
Project in Charge and Site Supervisor to ensure only certified signaller to
do signalling.
Safety Supervisor to caution other workers in the work area of the
hoisting operator or verbal warning.

6. MARKING OUT OF TENDON LOCATIONS

For activities identified with limbs cut for personnel, the following procedures
are to be carried out:

personal protection equipment prior to work.

For activities identified with personal cut by cutting tools, the following
procedures are to be carried out:
All personnel to disconnect electric power when equipment not in used.
All personnel must wear personnel protective equipment.
Only trained personnel is allowed to cut the ducting
Additional Procedures
All personnel are to take their own initiative to work safely on site:
Site Supervisor to train and educate the personnel on site through
toolbox meeting.
Hazards Identified

Cut by strand dispenser during handling
Hand crush between strand dispenser and strand coil
Strand sprung up and hit into personnel when cutting tie bands
For activities identified with hand crush between strand dispenser and strand
coil, the following procedures are to be carried out:
Project in Charge and Site Supervisor to ensure proper communication
when installing strand dispenser.
Project in Charge and Site Supervisor to ensure sufficient lighting to be
provided in the work area when working at night.
For activities identified with strand sprung up and hit into personnel when
cutting tie bands, the following procedures are to be carried out:
Site Supervisor and Safety Supervisor are to ensure personnel away
from the strand coil loose end when cutting the strand.
All personnel to wear safety gloves and long sleeve shirt to prevent
body exposure to the strand.
Additional Procedures
All personnel are to take their own initiative to work safely on site:
All personnel shall be reminded of the procedures during Toolbox
Meetings.

Hit by strand when cutting strand hazard

For activities identified with hit by strand when strand cutting hazard, the
following procedures are to be carried out:
Project in Charge and Site Supervisor are to use table saw when
carrying out pre-cutting.
All personnel are to wear safety gloves and long sleeve shirt when
exposed to tools.
Project in Charge and Site Supervisor to ensure that personnel are away
from the strand coil loose when cutting the strand.
For activities identified with hit by sparks when strand cutting hazard, the
following procedures are to be carried out:
Project in Charge and Site Supervisor to ensure no personnel other than
the operator are away during strand cutting.
All personnel are to wear safety gloves and long sleeve shirt when
exposed to spark.
For activities identified with fire hazard due to sparks hazard, the following
procedures are to be carried out:
Project in Charge and Site Supervisor to ensure that the area is free
from flammable material before cutting works commencing.
For activities identified with electrocution hazard, the following procedures are
to be carried out:
Project in Charge and Site Supervisor to check regularly on electric tools
and replace any damaged electrical tools.
Project in Charge and Site Supervisor to ensure that all electric tools are
checked by LEW.
Project in Charge and Site Supervisor to ensure that DB Box must be use
to connect power points.
Project in Charge and Site Supervisor to check that all electrical cables
are suspend 2m height from the ground level.

11. PUSHING OF STRAND INTO DUCTS
Hazards Identified
The following are the hazards identified:
Cut by strand hazard
Hit by strand during uncoiling strand hazard
Hit by strand at the tendon exit end hazard
Material fall from height hazard
For activities identified with hit by strand during uncoiling strand hazard, the
following procedures are to be carried out:
Project in Charge and Site Supervisor to ensure that personal are away
from the strand when cutting the tie bands.
All personnel are to wear safety gloves and long sleeve shirt when
carrying out the operation.
For activities identified with hot by strand at tendon exit end hazards, the
following procedures are to be carried out:
Project in Charge and Site Supervisor to ensure that the exit location is
barricade.
Project in Charge and Site Supervisor to deploy watchman at exit point
for lookout.
For activities identified with falling from height hazards, the following
procedures are to be carried out:
Project in Charge and Site Supervisor are to ensure that safety
guardrails and proper work access are available at the work place
before starting works.
Project in Charge and Site Supervisor are to ensure that there is
sufficient lighting at the work place before starting works.
Project in Charge and Site Supervisor to stop work if workplace found to
be unsafe until rectification work is carried out and completed by Main
Contractor.
All personnel working on the activity are to inform their direct
supervisors if there is insufficient lighting in their workplace. The Project
in Charge and Site Supervisor are to stop the works and inform the Main
Contractor to rectify lightings. Works can be resumed once the sufficient
lighting is provided.
Project in Charge and Site Supervisor to ensure that all personal to wear
safety harness and hook onto the life line when working on the mast
climbing platform.

12. PUSHING OF STRANDS USING ELECTRIC PUSHER
For activities identified with cut by strands during handling hazards, the
following procedures are to be carried out:
All personnel are to wear safety gloves and long sleeve shirt when
carrying out the operation.
For activities identified with falling from height hazards, the following
procedures are to be carried out:
Project in Charge and Site Supervisor are to ensure that safety
guardrails and proper work access are available at the work place
before starting works.
Project in Charge and Site Supervisor are to ensure that there is
sufficient lighting at the work place before starting works.
Project in Charge and Site Supervisor to stop work if workplace found to
be unsafe until rectification work is carried out and completed by Main
Contractor.
All personnel working on the activity are to inform their direct
supervisors if there is insufficient lighting in their workplace. The Project
in Charge and Site Supervisor are to stop the works and inform the Main
Contractor to rectify lightings. Works can be resumed once the sufficient

anchor heads and wedges quantities in the work area and consolidate
them at one area.
Site Supervisor and Safety Supervisor to barricade the work area below
to prevent other workers from exposing to the falling objects.

board at the edge of the platform.
For activities identified with electrocution hazard, the following procedures are
to be carried out:
Site Supervisor and Safety Supervisor to check regularly on electric
tools and replace any damaged electrical tools.
Site Supervisor and Safety Supervisor to ensure that all electric tools
are checked by LEW.
Site Supervisor and Safety Supervisor to ensure that DB Box must be
use to connect power points.

All personal must not stand at the back of the hydraulic jacks during
stressing at all times.
Site Supervisor and Safety Supervisor to barricade the stressing area to
prevent other personal from entering the stressing zone. Warning signs
to be set up to caution other personal.
For activities identified with electrocution hazard, the following procedures are
to be carried out:
Site Supervisor and Safety Supervisor to check regularly on electric
tools and replace any damaged electrical tools.
Site Supervisor and Safety Supervisor to ensure that all electric tools
are checked by LEW.
Site Supervisor and Safety Supervisor to ensure that DB Box must be
use to connect power points.
Site Supervisor and Safety Supervisor to check that all electrical cables
are suspend 2m height from the ground level.
For activities identified with bursting of hydraulic hoses hazard, the following
procedures are to be carried out:
Site Supervisor and Safety Supervisor to barricade the stressing area to
prevent other personal from entering the stressing zone. Warning signs
to be set up to caution other personal.
Only authorised personal wearing goggles are allowed to operate the
hydraulic jack and pump.
All hydraulic hoses to be checked and tighten before stressing
commence.

All loose material is to be placed at least 1 metre from the edge of the
structure.

may be exposed to falling material from this activity.
For activities identified with hot work hazards, the following procedures are to
be carried out:
Before starting works, personnel involved in the works is to ensure that
no flammable and combustible material are present in the vicinity.
All personnel are to wear safety eyewear, safety gloves and long sleeve
shirt when carrying out the operation.
Site Supervisor to ensure only operators are allowed to be at the cutting
location.
Site Supervisor to ensure that fire extinguisher is available at all times
during cutting.
Site Supervisor and Safety Supervisor to ensure fire watchman is
available at all times during cutting.
For activities identified with material falling from height hazard, the following
