PREPARING THE INTRAVENOUS BAG AND TUBING

PROCEDURE CHECKLIST 1 2 3 4 5 PE
1. Assessment:
a. Check the physician’s or qualified
practitioner’s order for the IV to be infused
and rate of flow to ensure accurate
administration.
b. Review information regarding the solution
and nursing implications in order to
administer the solution safely.
c. Assess the patency of the IV to ensure that
the solution will enter the vein and not the
surrounding tissue.
d. Assess the skin at the IV site so that the
solution will not be administered into an
inflamed or edematous site, which could
cause injury to the tissue.
e. Assess the client’s understanding of the
purpose of the IV infusion so that teaching
can be tailored to the client’s needs.
2. Check the physician’s or qualified practitioner’s
order for the IV solution.
3. Wash your hands
4. Check the client’s identification bracelet.
5. Be sure the tape at the IV site is loosened so it is
easier to change the tubing. Place a towel under
the arm of the IV site where the tubing will be
changed in order to keep the linen clean in case
of blood leaking from the needle during IV tubing
change.
6. Prepare a new bag by removing the protective
cover from the bag or bottle. Check the
expiration date on the bag and assess the bag
for cloudiness or leakage.
7. Open a new infusion set. Unroll the tubing and
close the roller clamp.
8. Spike the bag with the tip of the new tubing and
compress the drip chamber to fill halfway.
9. Open the roller clamp, remove the protective cap
from the end of the tubing, and flush the tubing
with solution.
10.Close the roller clamp and replace the cap
protector.
11.Apply clean gloves.
12.Remove the old tubing and replace with the new
tubing:
a. Place a sterile 2 x 2 gauze under the IV
catheter or heparin lock.
b. Stabilize the hub of the catheter or needle
and gently pull out the old tubing.
c. Quickly insert the new tubing into the hub of
the catheter or needle.
d. Open the roller clamp to establish flow of the
IV solution.
e. Reestablish the drip rate.
f. Apply new dressing to IV site.
13.Discard the old tubing and IV bag.
14.Remove the gloves and discard with all used
materials.
15.Apply a label with the date and time of change to
the tubing.
16.Wash your hands.
17.Clients should be instructed to report any
swelling or pain at the IV site. The client should
be instructed to notify the nurse if any leaking
from the tubing occurs
18.Documentation:
a. Date, time and name of IV solution stated
b. Date and time IV tubing was changed
