This risk assessment form evaluates the risks associated with an offsite visit to a theme park. It identifies potential hazards such as large crowds, becoming separated from the group, physically challenging rides, confrontation with the public, transportation, weather, and medical emergencies. Control measures are outlined to mitigate these risks, including dividing large groups into smaller units, conducting headcounts, briefing students on safety procedures and required behavior, and establishing an emergency contact plan. The form requires reviews to be conducted if circumstances change.
