Instructions to UP Car Sticker Applicants 1. Secure application form at the Office of the Vice Chancellor for Community Affairs (OVCCA). 2. Fill-up and submit accomplished form and documentary requirements for verification. 3. Pay the corresponding fee. 4. Claim your car sticker/s on the same day at the same place. 5. A staff will assist you attach the sticker on the drivers lower side of the front windshield. 6. The car sticker is non-transferable and specifically for the vehicle described in the application. 7. Notify the OVCCA of change of license plate number, change of vehicle ownership and vehicle repair of alteration.

Guidelines for the Availment of U.P. Diliman Car Sticker for SY 2012-2013 1. UP personnel and students who choose NOT to get car stickers shall be allowed access through the University portals/gates (University Avenue, Ylanan, Jacinto, Velasquez, and Magsaysay) upon presentation of the U.P. identification cards. 2. Those who could not present valid U.P. identification cards/alumni cards/Barangay ID/car stickers will be required to pass through the University Avenue portal and leave behind a drivers license or a current ID card with picture with the guard upon entry/recover the surrendered drivers license or ID card in the same gate upon exit. 2. Applications forms for the SY 2012-2013 car sticker can be secured at the OVCCA starting June 1, 2012. Sticker/s can be claimed on the same day at the same place upon verification of the requirements and paying the prescribed fees. 3. Documents to be presented 4.1 Duly accomplished application form 4.2 LTO Car/Motorcycle Registration (in such cases, copy of deed of sale/company certification required) 4.3 LTO Official Receipt 4.4 Drivers License of Owner 4.5 Drivers License of Driver (if owner is not the driver) 4.6 Valid Form 5 (for students) 4.7 UP Identification Card 4.8 UPAA Alumni ID (for alumni) 4.9 Proof of residency (address in license must coincide with address in the Car Registration for UP legitimate residents) 4.10 Proof of contract/legal arrangement/document (for legitimate service providers/concessionaires/ contractors) 4. Issuance of stickers will only be allowed to (a) faculty, staff, students; (b) alumni; (c) residents who are awardee of a UP Housing unit; and (d) legitimate service providers (concessionaires, contractors, school service). 5. Drivers and/or passengers bearing photocopies of identification cards shall be refused entry. These cards shall be confiscated by checkpoint security guards and shall be turned over to the Office of the Vice Chancellor for Community Affairs. Meanwhile, appropriate sanctions shall be imposed on university personnel and students who allow reproduction of their UP identification cards for purposes of gaining access through the university portals. 6. Sale of car stickers to outsiders shall be prohibited since it will defeat the objective of the University to prevent through traffic from using the University as an alternative route. It will also improve security on campus. The egress and ingress of vehicles shall be likewise monitored. 7.
