Statement of a concern or problem and expected outcomes


When the individual is the client Nursing Diagnosis: Ineffective airway clearance related to asthma as manifested by respiratory rate of 28 and wheezes in all lung fields Expected Outcomes: The client will demonstrate a cessation of wheezes in any lung

When the School district is the community Problem statement: The number of children between the ages of 5 and 18 with asthma in the school district increased from 3/1000 in March 2007 to 6/1000 in March 2008. Expected Outcomes: Reduce the number of children between the ages of 5 and 18 in the school district with the diagnosis of asthma from 6/1000 in 2007 to 3/1000 by year 2010.

When the County is the community Community Problem: The infant mortality rate for normal dale county was 11/1000 births in 2005 compared to the district infant mortality rate of 8/1000 and the national rate of 10/1000 Expected Outcomes: Reduce the infant mortality rate for normal dale county to 7/1000 births by 2010.

When the Hospital is the community

Community Problem: In March 2005, at normal dale County Hospital, 65 % Of the nursing staff washed their hands between clients. The recommended percentage in 90 %. Expected Outcomes: By March 2007, 85 % of the nursing staff wash their hands between clients.

The expected outcomes is compared to the Outcomes achieved at the end of the established time frame. Evaluation is not and end point. It ussually begins the assessment step of the next phase of community assessment
