Description:
On 17-June-2014 at 08.30 am, While the concrete mixer machine was
reversing near to the pit for concrete pouring , it just hit the hard
barrication of the excavated pit due to the miss communication
between driver and banks man.

Immediate Cause
1. More than one person gave signal to the driver.
2. Banks man didnt wore the visibility jacket to identify him
3. Lack of supervision.

Corrective Action

1. Driver was instructed to stop
the vehicle immediately
2. Special instruction given to the
Drivers And Banks man

Description:
On 23-june-2014 at around 04.00 pm, during road crossing excavation for the
duct bank, small piece of asphalt got detached & fell into the excavated pit .
A labor was narrowly escaped from being hit by it.

Immediate Cause
1. Some of the Asphalt debris were kept near to the Excavation
2. Improper House keeping
3. Poor supervision

1. Work was stopped at the same
time, and people were deployed
for removal of debris
2. Supervisor and the workers were
strictly instructed to place all the
excavated material and debris 1
meter away from Excavation

1. Insufficient Sign board At the Connection Champer 4 Excavation.

Action taken :
a. Sufficient number of sign boards of excavation and confined space
hazards were placed around the Excavated pit.
b. Supervisor is strictly instructed to ensure the availability of sign board
in every excavated areas
c. Importance of sign boards were clearly explained to Supervisors and
workforce
