per the specification laid by BP, Halliburton must
have take care of.

to blame?
The major issue within this case is the

blowout preventer.
Both BP and Transocean executives
approved of the installation of this device
to instill some form of safety compliance for
the workers.
Workers informed the stakeholders of the oil

safety faulty equipment and no immediate


repair action took place.
Executive decisions were not made for the
safety of the workers, but instead for profit.

Cutting Corners in the overall


construction, maintenance etc..
BP fabricated the recommendations on wildlife

contamination to get the approval of the


surrounding communities.
BP and Transocean Supervisors failure to
properly interpret pressure test for the well.
Oil rig safety test instructed by both
executive parties.
Safety tests Prompted by BP executives due to
reckless decisions for profit.
Blatant ignorance towards the severity of the faulty

oil safety equippment.

Could anything have been done to


prevent this disaster?
Prevention couldve been initiated

through:
Immediate action to revamp
faulty oil rig safety equipment.
Due to severity of the issue, all
production shouldve been shut
down until further notice.
Multiple equipment compliance
with safety guidelines shouldve
been checked weekly with BP and
