SBO – Safe Behaviour Observation

FINDINGS
Poor housekeeping by Timber team,
scattered and dismantled formworks
around the working area.
Unsafe act- worker climbed on
vertical reinforcement steel instead of
scaffold /ladder.
Unsafe scaffold- lack of horizontal tie
off, ‘U’ Head as scaffold base, lack of
hand rail and edge protection.
Working at unprotected edge without
providing edge protection.
IMMIDIATE ACTION
Upon several reminder violation
issued and followed with
clearance.
Offender called down from the
height and briefed on safe work
procedure.
Workers gathered for onsite
scaffold training.
